 1- NEED RECORDED NOTICE OF COMMENCEMENT. SEC. 713.13
 FLORIDA STATUTE.
 NOTE: THIS MAY BE RECORDED AT TIME OF PERMIT PICK UP.
  
 ***RESPONSE: NOTICE OF COMMENCEMENT SUBMITTED WAS
 RECORDED ON 12/12/2008. IT EXPIRES AFTER 90 DAYS WHEN
 WORK IS NOT STARTED. SEC. 713.13 FLORIDA STATUTE. NEED
 TO PROVIDE RE-RECORDED ONE.
  
 2- PLANS TO SPECIFY ALL LIVE AND DEAD LOADS AS REQUIRED
 BY SEC. R301.1 AND TABLE R301.5 OF 2004 FLORIDA
 RESIDENTIAL CODE. NOTE: ATTIC LIVE LOADS NEED TO BE
 INCLUDED ALSO.

 ARE VERY CONFUSING AND STILL ARE NOT ADDRESSING THE
 REQUIRED ATTIC LIVE LOADS REQUIRED BY TABLE R301.5 OF
 2004 FLORIDA RESIDENTIAL CODE.
 NOTE: ONE SET OF PLANS WAS RETAINED IN THIS OFFICE TO
 SEND TO BOARD OF ENGINEERS FOR REVIEW.
  
 3- PROVIDE 2 COPIES OF PRODUCT APPROVALS AS REQUIRED BY
 DCA RULE 9B-72 FOR:
 A) PRECAST LINTELS AS SHOWN ON PLANS.
  
 ***RESPONSE:
 NOA #03-0605.04 SUBMITTED EXPIRED ON 09/11/2008 AND IS
 FOR 4", 6" AND 12" PRECAST LINTELS. NONE OF THESE SIZES
 ARE USED ON PLANS. PROVIDE PRODUCT APPROVAL FOR 8"
 LINTELS. ALSO, ENGINEER OF RECORD TO REVIEW AND APPROVE
 IN WRITING NEW PRODUCT APPROVAL.
  
 B) ENTRY DOOR AND KITCHEN AND BEDROOM EXTERIOR DOORS.
 CLEARLY IDENTIFY WHICH PRODUCT APPROVAL IS FOR THE
 ENTRY AND ALL OTHER OPENINGS.
 ***NOTE: NOA 02-1211.10 SUBMITTED EXPIRED ON JULY 17,
 2008. NEED TO PROVIDE CURRENT PRODUCT APPROVAL.

 (1) NOA 04-1022.04 SUBMITTED IS FOR "INSWING" DOORS.
 PLANS SHOW "OUTSWING" DOORS. REVISE AS REQUIRED.
  
 (2) MISSING BEDROOM EXTERIOR DOOR.
  
 (3) PRODUCT APPROVAL SUBMITTED DOESN'T MATCH TYPE OF
 DOOR SHOWN ON DOOR SCHEDULE AND EXTERIOR ELEVATIONS.

 (4) GLASS DON'T HAVE IMPACT PROTECTION. SHUTTERS ARE

 (6) PRODUCT APPROVAL SUBMITTED DOESN'T MEET THE DESIGN
 PRESSURES CALCULATED BY THE ENGINEER. REVISE AS
 REQUIRED.
  
 C) HURRICANE SHUTTERS: PROVIDE COMPLETE INSTALLATION
 SCHEDULE. SEE ATTACHED SAMPLE. COMPLETE ALL THE
 INFORMATION AND CLEARLY MARK ON SHUTTERS DRAWINGS
 SPECIFIC MOUNTING CONDITIONS (EG: WALL MOUNTED, BUILD
 OUT, ETC) AND SPECIFIC ANCHOR TYPE AND SPACING TO BE
 USED.
  
 ***RESPONSE:
 THIS ITEM WAS NOT ADDRESSED AT ALL. PROVIDE COMPLETE
 INFORMATION AND CLARIFY WHY SUBMITTING TWO DIFFERENT
 PRODUCT APPROVALS FOR TWO DIFFERENT MANUFACTURER'S.
 ALSO, PLEASE MAKE SURE TO PROVIDE TWO COPIES FOR EITHER
 SELECTION. EG: ONLY ONE COPY OF NOA 06-0706.17 WAS
 PROVIDED. AND, PRODUCT APPROVAL NEEDS TO BE REVIEWED
 AND APPROVED BY THE ENGINEER OF RECORD.
  
 4- ENGINEER OF RECORD TO REVIEW AND APPROVE IN WRITING
 (NOT SIGNING AND SEALING) ALL PRODUCT APPROVALS AS
 REQUIRED BY SEC. 106.3.3 CITY AMENDMENTS TO FBC 2004.
  
 ***RESPONSE: SOME PRODUCT APPROVALS SUBMITTED WERE NOT
 APPROVED BY THE ENGINEER.
  
 5- SINCE THE ENERGY CALCULATIONS ARE TAKEN THE CEILING
 FAN CREDIT, THEN THE ELECTRICAL PLAN NEEDS TO SHOW SIZE
 OF CEILING FANS AS REQUIRED BY TABLE 13-607.1.A.1 OF
 FBC 2004. AND, THEY NEED TO BE INSTALLED FOR
 INSPECTION.
