 1. A PERMIT HAS ALREADY BEEN ISSUED FOR 08030015, SAME
 ADDRESS. PLEASE EXPLAIN WHY A SEPARATE PERMIT HAS BEEN
 APPLIED FOR. IF THERE ARE TWO PERMITS FOR THE SAME
 SCOPE OF WORK, A LETTER SHOULD BE SUBMITTED REQUESTING
 THAT ONE BE VOIDED.
  
 2. THE LETTER SUBMITTED IS NOT SEALED. IN ORDER TO BE A
 PART OF THE PERMIT DOCUMENTS, THIS LETTER IS TO BE IN
 COMPLIANCE WITH FAC61G15-23, AND NEEDS TO INCLUDE AN
 IMPRESSED SEAL AS WELL AS THE CERTIFICATE OF
 AUTHORIZATION. THE LETTER IS REQUIRED OR HAVE THE
 INFORMATION INCORPORATED INTO THE SPECIFICATION.
  
 3. THE DRAWING INDEX REFERS TO TWO DETAILS, SK-1 AND
 SK-2, BUT I WAS UNABLE TO LOCATE THESE PAGES IN THE
 DOCUMENTS SUBMITTED.
  
 4. PROVIDE A TENDON LAYOUT PLAN SHOWING LOCATION OF
 REPAIR. THE SPECIFICATION STATES 4TH FLOOR. IS THIS
 LOCATED WITHIN A UNIT OR IN THE COMMON AREA?
  
 5. THE ENGINEER STATES THAT AN "EXPEDITIOUS REPAIR" IS
 RECOMMENDED, BUT THE SPECIFICATION IS DATED FEBRUARY
 15, 2008, ALMOST A YEAR AGO. IS THE STRUCTURAL
 INTEGRITY OF THE BUILDING AFFECTED?
  
 6. THE SPECIFICATION IS TO BE IN COMPLIANCE WITH
 FAC61G15-23.002. THE INDEX SHEET DOES NOT CONTAIN A
 TITLE BLOCK WITH ALL REQUIRED INFORMATION.
  
 7. PREINSPECT REQUIRED PER KEN CONRAD. THE PLANS HAVE
 BEEN ROUTED TO K. CONRAD. 561-805-6666
  
 8. AS THIS IS A THRESHOLD BUILDING, A THRESHOLD
 INSPECTOR MAY BE REQUIRED AFTER SCOPE/LOCATION OF WORK
 HAS BEEN DETERMINED.
