 1- NEED RECORDED NOTICE OF COMMENCEMENT. SEC. 713.13
 FLORIDA STATUTE.
 NOTE: THIS MAY BE RECORDED AT TIME OF PERMIT PICK UP.
  
 2- NEED TO BRING COPY OF PAID RECEIPT FOR PALM BEACH
 COUNTY IMPACT FEES (REPEAT COMMENT). NO COPY SUBMITTED
 AT THIS TIME. NEED COPY OF PAID RECEIPT AND PLANS WITH
 PALM BEACH COUNTY IMPACT FEES STAMP FOR RECORD KEEPING.
  
 3- ELEVATOR & STAIR WALL SECTION: CLEARLY SPECIFY
 FASTENING OF WALL TO SUPPORT ELEVATOR RAIL. "GLUE AND
 SCREW TO WALL FRAMING" ARE NOT ADEQUATE SPECIFICATIONS.
 NEED TO SPECIFY SCREWS SIZES AND SPACING. SEC. 106.1.2
 CITY AMENDMENTS TO FBC 2004 TO VERIFY COMPLIANCE WITH
 SEC. R301 OF 2004 FLORIDA RESIDENTIAL CODE.
  
 4- ADDED HAND WRITTEN INFORMATION ON SIGNED AND SEALED
 DRAWINGS ARE NOT ALLOWED. ANY REVISIONS NEED TO BE DONE
 ON ORIGINAL DRAWINGS. REVISE ALL ORIGINAL DRAWINGS
 SHOWING ALL REVISIONS ADDRESSING THE COMMENTS AS
 REQUIRED AND RE-SUBMIT SIGNED AND SEALED COPIES AS
 REQUIRED BY CHAPTER 481 FLORIDA STATUTES AND SEC. 61G15
 FLORIDA ADMINISTRATIVE CODE.
  
 5- PROVIDE 2 COPIES OF PRODUCT APPROVALS AS REQUIRED BY
 DCA RULE 9B-72 FOR:
 A) THERMA-TRU FIBERGLASS DOORS. FL-5262 SUBMITTED IS
 FOR 2007 CODE VERSION. CURRENT CODE VERSION IS 2004.
 ALS0: DOOR WITH GLAZING REQUIRES IMPACT PROTECTION.
 SPECIFY IS USING GLASS DOOR AND PROVIDE INFORMATION FOR
 IMPACT PROTECTION AS REQUIRED.
  
 B) SEALOFLEX WATERPROOFING. PROVIDE THE FOLLOWING
 INFORMATION FOR NOA 05-0901.04 SUBMITTED:
 (A) SELECT FROM PAGES 4 THRU 9 SPECIFIC ASSEMBLY TO BE
 INSTALLED.
  
 (B) PROVIDE RAS 117 CALCULATIONS FOR NAIL ENHANCING AT
 PERIMETER AND CORNER AREAS AS REQUIRED BY GENERAL
 LIMITATION #7. THESE CALCULATIONS NEED TO BE SIGNED AND
 SEALED AS REQUIRED BY GENERAL LIMITATION #7 ON LAST
 PAGE OF NOA SUBMITTED. (NOTE: KEEP IN MIND EXPOSURE "C"
 WHEN DOING RAS 117 CALCULATIONS).
  
 (C) PROVIDE GARAGE DOORS PRODUCT APPROVAL FOR 2004 CODE
 VERSION. INFORMATION SUBMITTED IS FOR 2007 CODE
 VERSION. ALSO, PROVIDE COMPLETE PRODUCT APPROVAL
 PACKAGE, INCLUDING THE FLORIDA PRODUCT APPROVAL COVER
 PAGE.
  
 6- PLEASE, MAKE SURE ALL PRODUCT APPROVAL ARE REVIEWED
 AND APPROVED BY THE ARCHITECT OF RECORD PRIOR TO
 SUBMITTAL TO THIS OFFICE. SEC. 106.3.3 CITY AMENDMENTS
 TO FBC 2004.
  
 7- INFORMATION SUBMITTED FOR DRIVEWAYS AND DRIVEWAY
 APPROACH ARE DONE UNDER SEPARATE PERMITS. APPLY FOR
 THOSE PERMITS AS REQUIRED. CONTACT ZONING (561)822-1435
 AND ENGINEERING (561) 494-1040 FOR THEIR REQUIREMENTS.
  
 8- PROPERTY IS LOCATED ON FLOOD ZONE A5. ELEVATION
 CERTIFICATE WILL BE REQUIRED PRIOR TO GETTING THE
 CERTIFICATE OF OCCUPANCY (C.O.) SHOWING MINIMUM FINISH
 FLOOR ELEVATION OF 7.5'.
  
 9- OWES $966.54 FOR UPDATED VALUATION AND RADON FEES
