| | NOT MATCH THE LANDSCAPE PLAN APPROVED BY THE ZONING |
| | BOARD OF APPEALS ON SEPTEMBER 5, 1999. PLEASE NOTE THAT |
| | IF AN UPDATED LANDSCAPE PLAN WAS APPROVED FOR THIS SITE |
| | SUBSEQUENT TO SEPTEMBER 7, 1999, A COPY SHALL BE |
| | SUBMITTED WITH THIS BUILDING PERMIT APPLICATION. |
| | OTHERWISE, LANDSCAPING ON SITE SHALL MATCH THE APPROVED |
| | LANDSCAPE PLAN, OR A NEW LANDSCAPE PLAN SHALL BE |
| | REVIEWED AND APPROVED BY THE PLANNING AND ZONING |

| | REFUSE AND DEBRIS. TREES ON THE SUBJECT PROPERTY SHALL |
| | BE CORRECTIVELY TRIMMED BY A CERTIFIED ARBORIST TO |
| | PRESENT A GOOD AND HEALTHY CONDITION. |

| | C. THE SITE PLAN APPROVED BY THE ZONING BOARD OF |
| | APPEALS ON SEPTEMBER 5, 1999 SHOWED A TOTAL OF 10 |
| | REQUIRED AND PROVIDED PARKING SPACES. HOWEVER, THE |
| | SUBJECT PROPERTY ONLY HAS 7 PARKING SPACES. THE 3 |
| | PARKING SPACES THAT WERE REMOVED ARE UNDER THE TENT. |
| | THE REQUIRED 10 PARKING SPACES SHALL BE MAINTAINED ON |
| | SITE. |

| | D. PER THE FINAL ORDER DATED SEPTEMBER 5, 1999, ?THE |
| | EXTERIOR PAY PHONE ALONG SOUTH DIXIE HIGHWAY SHALL BE |
| | REMOVED; NO FUTURE OUTDOOR PAY PHONES SHALL BE |
| | INSTALLED.? THERE IS A NEW PAY PHONE LOCATED ALONG |
| | HUNTER STREET, WHICH WAS NEVER SHOWN ON THE APPROVED |
| | SITE PLAN. THEREFORE, THIS PAY PHONE AND ANY OTHER |
| | EXTERIOR PAY PHONES ON THE SITE SHALL BE REMOVED. |

| | E. PURSUANT TO SECTION 94-444(B), ALL MECHANICAL |
| | EQUIPMENTS SHALL BE SCREENED BY WALLS AND FENCES. THE |
| | MECHANICAL EQUIPMENT LOCATED ON THE SOUTHEAST CORNER OF |
| | THE SITE SHALL BE SCREENED WITH A WALL AND A HEDGE AS |
| | REQUIRED PER THIS SECTION. |

| | 4. VIOLATIONS LISTED IN COMMENT NO. 3 ABOVE SHALL BE |
| | ADDRESSED PRIOR TO OBTAINING A CERTIFICATE OF |
| | COMPLETION/OCCUPANCY FOR THIS PERMIT. |
