 OF PAID RECEIPT IF FEES ARE DUE.
  
 3- PROVIDE SOIL TEST REPORT. SEC. R401.4 OF 2004
 FLORIDA RESIDENTIAL CODE.
  
 4- PERMIT VALUATION WAS UPDATED TO $65,321 AS REQUIRED
 BY SEC. 108.3 CITY AMENDMENTS TO FBC 2004.OWES
 $1,171.18 FOR PERMIT VALUATION AND RADON FEES.

 APPLICATION. ALL FES NEED TO BEPAID AT TIME OF

 13-101.2.2 OF FBC 2004.

 6- PROVIDE PRODUCT APPROVAL (2 COPIES)AS REQUIRED BY

 A) ENTRY DOOR WITH SIDELITE ASSEMBLY AS SHOWN ON
 PLANS AND EXTERIOR ELEVATIONS. THERE WERE TWO
 DIFFERENT DOORPRODUCT APPROVALS SUBMITTED AND NONE OF
 THEM HAVE SIDELITE.NOA 02-1211.10 AND NOA
 07-0731.04.PROVIDE 2 COPIES OF PRODUCT APPROVAL FOR
 EACH DOOR MODEL.ONE FOR THE ENTRY DOOR AND OTHER FOR
 THE REAR DOOR.CLEARLY MARK WHICH DOOR IS THE ENTRY
 DOOR AND WHICH IS THE REAR DOOR.
