 REVISIONS.
  
 NOTE: PLANS SUBMITTED ARE COMPLETELY DIFFERENT THAN
 THOSE SUBMITTED THE FIRST TIME.
  
 1- PROVIDE RECORDED NOTICE OF COMMENCEMENT. SEC. 713.13
 FLORIDA STATUTE.
 ***NOTE: THIS MAY BE RECORDED LATER AT TIME OF PERMIT
 PICK UP.
  
 2- PERMIT VALUATION WAS UPDATED AS REQUIRED BY SEC.
 108.3 CITY AMENDMENTS TO FBC 2004. OWES $346.78 FOR
 UPDATE VALUATION AND FOR RADON FEES.
  
 3- PROVIDE 2 COPIES OF PRODUCT APPROVALS AS REQUIRED BY
 DCA RULE 9B-72 FOR:
 A) FLAT ROOF ASSEMBLY FOR ROOF TOP PATIO. AND, CLEARLY
 IDENTIFY SPECIFIC APPROVED ASSEMBLY TO BE INSTALLED.
 PROVIDE RAS 117 CALCULATIONS FOR NAIL ENHANCING AT
 PERIMETER AND CORNER AREAS AS REQUIRED BY GENERAL
 LIMITATION #7 THAT IS TYPICALLY SHOWN ON SELECTED
 ASSEMBLY. CHECK IF CALCULATIONS NEED TO BE SIGNED AND
 SEALED BY ARCHITECT OR ENGINEER, IF THEY DO, THEN THEY
 NEED TO BE PROVIDED. CHECK GENERAL LIMITATION ON LAST
 PAGE OF PRODUCT APPROVAL FOR REQUIREMENTS.
 NOTE: IF SELECTED ASSEMBLY IS LIMITED BY GENERAL
 LIMITATION #9, THEN IT MAY NOT BE USED IN THIS AREA
 BECAUSE IT WILL NOT MEET THE REQUIRED DESIGN PRESSURES
 OF THIS AREA.
  
 B) SECOND FLOOR PATIO #2 ASSEMBLY THAT RECEIVES THE
 FLOOR TILE. CHECK FOR UPLIFT RESISTANCE AND PROVIDE
 REQUIRED INFORMATION. EG: RAS 117 CALCULATIONS. SEE
 COMMENTS ON ITEM 3A ABOVE.
  
 C) GARAGE DOOR. PROVIDE PRODUCT APPROVAL FOR GARAGE
 DOOR SIZE AS SHOWN ON PLANS. PRODUCT APPROVAL FL9462-R0
 SUBMITTED IS FOR 16' MAX. WIDE DOORS. THERE IS A
 SEPARATE PRODUCT APPROVAL FOR SMALLER DOORS.
  
 D) (CAST-CRETE) 4", 6" & 12" PRECAST LINTELS. NOA
 03-0605.04 SUBMITTED EXPIRED ON 09/11/2008. NEED TO
 PROVIDE CURRENT ONE.
  
 E) CASEMENT WINDOWS. NOA 05-1129.11 SUBMITTED EXPIRED
 ON MAY 22, 2008. NEED TO PROVIDE CURRENT ONE.
  
 4- ENGINEER OF RECORD TO REVIEW AND APPROVE IN WRITING
 (NOT SIGNING AND SEALING) ALL PRODUCT APPROVALS AS
 REQUIRED BY SEC. 106.3.3 CITY AMENDMENTS TO FBC 2004.
 NEED TO REVISE LIST OF PRODUCT APPROVALS SHOWN ON PLANS
 AND ADDITIONAL LIST PROVIDED TO INCLUDE THE ADDITIONAL
 PRODUCT APPROVALS AND REVISE EXPIRED ONES. ALSO, LIST
 IS MISSING METAL ROOFING, FLAT ROOF, ETC.
  
 NOTE: PERSON TO REVIEW AND APPROVE THE PRODUCT APPROVAL
 IS THE ENGINEER WHO SIGNED AND SEALED THE PLANS. HE IS
 THE DESIGNER OF RECORD RESPONSIBLE FOR THESE PLANS.
  
 5- SHEET A2.0: REVISE ATTIC WITH STORAGE LIVE LOAD
 SHOWN ON CONSTRUCTION NOTE #12. SEE TABLE R301.5 OF
 2004 FLORIDA RESIDENTIAL CODE AND REVISE AS REQUIRED.
  
 6- SHEETS A9.1, A9.2 AND A9.3 NEED TO BE SIGNED AND
 SEALED BY THE ENGINEER OF RECORD. ALSO, PROVIDE HIS
 TITLE BLOCK AND INFORMATION ON THESE SHEETS.
  
 7- A) PROVIDE COMPLETE INFORMATION FOR INSTALLATION OF
 SPIRAL STAIRCASE TO SHOW COMPLIANCE WITH THE DESIGN
 LOADS OF SEC. R301 OF 2004 FLORIDA RESIDENTIAL CODE.
 SPECIFY ALL CONNECTIONS AT TOP AND BOTTOM.
  
 B) PROVIDE COMPLETE SPECIFICATIONS AND CUT SHEETS FOR
 SPECIFIC SPIRAL STAIRCASE TO BE INSTALLED TO SHOW
 COMPLIANCE WITH SEC. R311.5.8 OF 2004 FLORIDA
 RESIDENTIAL CODE.
  
 8- PROVIDE COMPLETE CONSTRUCTION DETAILS/SECTIONS AND
 SPECIFICATIONS FOR HANDRAILS AND GUARDRAILS TO VERIFY
 COMPLIANCE WITH TABLE R301.5 (AND FOOTNOTES D & F),
 SEC. 312 OF 2004 FLORIDA RESIDENTIAL CODE. MISSING
 CONSTRUCTION OF STAIRS AND RAILINGS TO RESIST LOADS
 REQUIRED BY TABLE R301.5, SPACING BETWEEN PICKETS, ETC.
 STAIRWAY SECTION ON SHEET A9.2 IS MISSING THIS
 INFORMATION AND THERE IS NOT INFORMATION GIVEN FOR 2ND
 AND 3RD FLOOR GUARDRAILS.
  
 9- PROVIDE STRUCTURAL BUCK DETAILS FOR GARAGE DOOR AND
 FOR ALL OTHER DOORS AND WINDOWS. SEE PRODUCT APPROVALS
 FOR SIZE OF BUCK REQUIRED AND PROVIDE FULL SUPPORT TO
 THE DOOR AND WINDOW FRAME AS REQUIRED BY SEC. R613.6 OF
 2004 FLORIDA RESIDENTIAL CODE. NO INFORMATION PROVIDED.
  
 10- SURVEY NEEDS TO HAVE SURVEYOR'S ORIGINAL SIGNATURE
 (NOT COPY) AS REQUIRED BY CHAPTER 472 FLORIDA STATUTE.
