 3) THE CLAIMED VALUATION ON THE PERMIT APPLICATION IS
 LOW. FOR PERMITTING PURPOSES, VALUATION OF BUILDINGS
 AND SYSTEMS SHALL BE THE TOTAL REPLACEMENT COST
 EXCLUDING LAND VALUE. OUR VALUATION REFERENCE IS ICC
 (BVD), MARSHALL-SWIFT AND MEANS COST ANALYSIS SERVICES
 PER 108.3 FBC*
  
 4) THE STATE OF FLORIDA REQUIRES COLLECTION OF A RADON
 SURCHARGE ON ALL NEW CONSTRUCTION COMPUTED ON UNDER
 ROOF FLOOR SPACE PER 10D-91 FAC. THESE FEES WILL BE
 ADDED TO THE PERMIT APPLICATION FEES
  
 NOTE: FEES ARE DUE IN THE AMOUNT OF $731.76 FOR
 INCREASED VALUE AND STATE RADON FEES THIS AMOUNT IS

 7) SUBMIT TWO COPIES OF ENERGY CALC'S PER 13-101.2.2
 FBC. BE SURE THAT THEY ARE SIGNED AND DATED BY PREPARER
 AND OWNER/AGENT PRIOR TO SUBMITTAL. NOTE: WHOLE HOUSE
 METHOD A WAS CHOSEN PLEASE CORRECT THE AS-BUILT
 CALCULATIONS FOR THE GLAZED OPENINGS TO REFLECT ACTUAL
 CONDITIONS ON THE DRAWINGS, AMOUNT OF OPENINGS,
 ORIENTATION, OVERHANG LENGTH AND HEIGHT, ETC.
 ********ADDITION CONTAINS 5 WINDOWS BUT 2 ARE ON THE
 NORTH SIDE, 3 ON THE WEST AND 2 ARE ON THE SOUTH, THIS
 IS NOT WORST CASE THEREFORE THE SIZES/AREAS SHOULD
 MATCH ACTUAL LAYOUT, ALSO THE HEIGHT USED IS FROM THE
 SILL TO THE UNDERSIDE OF THE OVERHANG WILL NOT BE THE
 SAME ON DIFFERENT WINDOW SIZES THEREFORE THEY CANNOT
 ALL BE 1.3 FEET AS STATED**********
  
 9) THE PERMIT APPLICATION SHALL INCLUDE TWO COPIES OF
 EACH PRODUCT APPROVAL THAT IS RELEVANT PER 9B-72.
 *******NO PRODUCT APPROVALS HAVE YET BEEN SUBMITTED FOR
 GLASS BLOCK AND LINTELS AND REAR DOOR MIAMI-DADE NOA
 HAS EXPIRED. AS THE CITY OF WEST PALM BEACH NO LONGER
 REQUIRES THE STATE OF FLORIDA PRODUCT APPROVALS A NEW
 UPDATED MIAMI-DADE NOA WOULD BE ACCEPTABLE (SEE NOA
 #08-0516.07 AND IT USES THE SAME DRAWING

 10) THOSE PRODUCTS WHICH ARE REGULATED BY DCA RULE
 9B-72 (PRODUCT APPROVALS) SHALL BE REVIEWED AND
 APPROVED IN WRITING BY THE DESIGNER OF RECORD PRIOR TO
 SUBMITTAL FOR JURISDICTIONAL APPROVAL PER 106.3.3 FBC*
 *******ANY NEW PRODUCT APPROVAL SHALL BE REVIEWED AND
 APPROVED BY DESIGNER OF RECORD. PLEASE NOTE THAT THE
 SHUTTER APPROVALS UNDER THE FLORIDA COVER SHEETS
 FL722-R1 ARE NOT VALIDATED BY THE SUBMITTED MIAMI-DADE
 NOA, EITHER SUBMIT CORRECT DRAWING FOR THE FLORIDA
 APPROVAL OR REMOVE THE FLORIDA COVERS AND SUBMIT THE
 NOA BUT THE MIAMI-DADE REPORT WOULD NEED TO BE REVIEWED

 AND A COPY OF THE PAID RECEIPT ATTACHED TO THE PERMIT
