 INSTALL FIBERGLASS STRUCTURE
  
 DENIED BY BUILDING
  
 1) THE ENGINEER FOR SHELTER WORKS IS USING AN EMBOSSED
 SEAL THAT IS NO LONGER ACCEPTABLE AS OF JANUARY 1,
 2006. SEE FAC 61G15-23.001 AND F.S.471.025.
  
 2) ENGINEER IS REQUIRED TO LEGIBLY INDICATE THEIR NAME,
 ADDRESS AND LICENSE NUMBER ON EACH SHEET. SEE FAC
 61G15-23.002.
  
 3) THE WRONG CODES ARE SPECIFIED ON THE PLANS AND
 CALCULATIONS. THE CODE CURRENTLY IN EFFECT IS THE 2004
 FBC W/2007 REVISIONS.
  
 4) THE CALCULATIONS REFERENCE THE WRONG EDITIONS OF
 FBC, ASCE7 AND ACI 318. THE 2004 FBC REFERENCES
 ASCE7-02 AND ACI 318-02. PLEASE REVISE.
  
 5) GENERAL NOTE #2 ON SHEET 1 OF PLANS SPECIFIES THAT A
 MINIMUM SEPARATION DISTANCE OF 10 FEET IS REQUIRED FROM
 THE NEAREST BUILDING. THE SITE PLAN DOES NOT COMPLY.
  
 6) SPECIFY THE SIZE AND LENGTH/EMBED DEPTH OF THE
 EXPANSION ANCHORS @ 6" OC ON SHEET 4 OF THE PLANS. THE
 CALCULATIONS SPECIFY 3/8" DIAMETER ANCHOR BOLTS WITH
 1-1/2" EMBEDMENT SPACED 12" OC. SEE CALCULATION SHEET 7
 OF 13.
  
 7) SUBMIT TWO COPIES OF PRODUCT APPROVALS FOR THE
 EXTERIOR DOOR AS REQUIRED BY DCA RULE 9B-72.
  
 8) FBC 106.3* CITY AMENDMENTS PRODUCT APPROVALS. THOSE
 PRODUCT WHICH ARE REGULATED BY DCA RULE 9B-72 SHALL BE
 REVIEWED AND APPROVED IN WRITING BY THE DESIGNER OF
 RECORD PRIOR TO SUBMITTAL FOR JURISDICTIONAL APPROVAL.
  
 9) FBC TABLE 602 REQUIRES TWO HOUR FIRE RATED WALLS FOR
 A BUILDING OF TYPE V-B CONSTRUCTION AND F-1 OCCUPANCY
 AND A FIRE SEPARATION DISTANCE OF LESS THAN 5' . NOTE:
 SEE 702.1 DEFINITIONS. FIRE SEPARATION DISTANCE. THE
 DISTANCE MEASURED FROM THE BUILDING FACE TO THE CLOSEST
 INTERIOR LOT LINE, TO THE CENTERLINE OF A STREET, ALLEY
 OR PUBLIC WAY, OR TO AN IMAGINARY LINE BETWEEN TWO
 BUILDINGS ON THE LOT. THE DISTANCE SHALL BE MEASURED AT
 RIGHT ANGLES FROM THE FACE OF THE WALL.
  
 10) SPECIFY THE OCCUPANCY, CONSTRUCTION TYPE AND AREA
 OF WALL OPENINGS FOR THE ADJACENT BUILDING IN ORDER TO
 DETERMINE COMPLIANCE WITH FIRE SEPARATION REQUIREMENTS.
 FBC 702.
  
 11) THE MAXIMUM AREA OF WALL OPENINGS IN BOTH BUILDINGS
 IS REQUIRED TO COMPLY WITH FBC TABLE 704.8. NO OPENINGS
 ARE PERMITTED FOR A FIRE SEPARATION DISTANCE OF 0-3
 FEET.
  
 12) FBC 503.1.3 PERMITS TWO BUILDINGS ON THE SAME LOT
 TO BE CONSIDERED AS PORTIONS OF ONE BUILDING IF CERTAIN
 CONDITIONS ARE MET. SEE FBC 503.1.3 BUILDINGS ON SAME
 LOT.
 TWO OR MORE BUILDINGS ON THE SAME LOT SHALL BE
 REGULATED AS SEPARATE BUILDINGS OR SHALL BE CONSIDERED
 AS PORTIONS OF ONE BUILDING IF THE HEIGHT OF EACH
 BUILDING AND THE AGGREGATE AREA OF BUILDINGS ARE WITHIN
 THE LIMITATIONS OF TABLE 503 AS MODIFIED BY SECTIONS
 504 AND 506 . THE PROVISIONS OF THIS CODE APPLICABLE TO
 THE AGGREGATE BUILDING SHALL BE APPLICABLE TO EACH
 BUILDING. IF YOU CHOOSE TO DO THIS SHOW HOW THE TWO
 BUILDINGS WILL COMPLY.
  
 13) SPECIFY THE TYPE (LIQUID OR GAS) AND AMOUNT OF
 CHLORINE THAT WILL BE STORED IN THE BUILDING. SUBMIT
 MATERIAL SAFETY DATA SHEETS FOR THE PRODUCT.THIS
 INFORMATION TO BE USED TO DETERMINE IF THE OCCUPANCY
 WILL BE DETERMINED AS HIGH HAZARD GROUP H. SEE FBC
