 1) THE DESIGN ENGINEER SHALL SIGN AND SEAL ALL SHEETS
 OF THE SUBMITTED PLAN. THE TITLE SHEET WAS NOT SIGNED
 AND SEALED AS PER THE FOLLOWING:
  
 471.025SEALS.--
  
 (1)THE BOARD SHALL PRESCRIBE, BY RULE, ONE OR MORE
 FORMS OF SEAL TO BE USED BY LICENSEES. EACH LICENSEE
 SHALL OBTAIN AT LEAST ONE SEAL IN THE FORM APPROVED BY
 RULE OF THE BOARD AND MAY, IN ADDITION, REGISTER HIS OR
 HER SEAL ELECTRONICALLY IN ACCORDANCE WITH SS.
 668.001-668.006. ALL FINAL DRAWINGS, SPECIFICATIONS,
 PLANS, REPORTS, OR DOCUMENTS PREPARED OR ISSUED BY THE
 LICENSEE AND BEING FILED FOR PUBLIC RECORD AND ALL
 FINAL DOCUMENTS PROVIDED TO THE OWNER OR THE OWNER'S
 REPRESENTATIVE SHALL BE SIGNED BY THE LICENSEE, DATED,
 AND SEALED WITH SAID SEAL. SUCH SIGNATURE, DATE, AND
 SEAL SHALL BE EVIDENCE OF THE AUTHENTICITY OF THAT TO
 WHICH THEY ARE AFFIXED. DRAWINGS, SPECIFICATIONS,
 PLANS, REPORTS, FINAL DOCUMENTS, OR DOCUMENTS PREPARED
 OR ISSUED BY A LICENSEE MAY BE TRANSMITTED
 ELECTRONICALLY AND MAY BE SIGNED BY THE LICENSEE,
 DATED, AND SEALED ELECTRONICALLY WITH SAID SEAL IN
 ACCORDANCE WITH SS. 668.001-668.006.
  
  
 2) DUE TO THE COMPLEXITY OF THIS CONCRETE RESTORATION
 PROJECT AND THE MAGNITUDE OF THE PROPOSED SHORING
 INSTALLATION, THE FOLLOWING SHALL BE REQUIRED:
  
 105.14 PERMIT ISSUED ON BASIS OF AN AFFIDAVIT.
  
 WHENEVER A PERMIT IS ISSUED IN RELIANCE UPON AN
 AFFIDAVIT OR WHENEVER THE WORK TO BE COVERED BY A
 PERMIT INVOLVES INSTALLATION UNDER CONDITIONS WHICH, IN
 THE OPINION OF THE BUILDING OFFICIAL, ARE HAZARDOUS OR
 COMPLEX, THE BUILDING OFFICIAL SHALL REQUIRE THAT THE
 ARCHITECT OR ENGINEER WHO SIGNED THE AFFIDAVIT OR
 PREPARED THE DRAWINGS OR COMPUTATIONS SHALL SUPERVISE
 SUCH WORK. IN ADDITION, THEY SHALL BE RESPONSIBLE FOR
 CONFORMITY TO THE PERMIT, PROVIDE COPIES OF INSPECTION
 REPORTS AS INSPECTIONS ARE PERFORMED, AND UPON
 COMPLETION MAKE AND FILE WITH THE BUILDING OFFICIAL
 WRITTEN AFFIDAVIT THAT THE WORK HAS BEEN DONE IN
 CONFORMITY TO THE REVIEWED PLANS AND WITH THE
 STRUCTURAL PROVISIONS OF THE TECHNICAL CODES. IN THE
 EVENT SUCH ARCHITECT OR ENGINEER IS NOT AVAILABLE, THE
 OWNER SHALL EMPLOY IN HIS STEAD A COMPETENT PERSON OR
 AGENCY WHOSE QUALIFICATIONS ARE REVIEWED BY THE
 BUILDING OFFICIAL. THE BUILDING OFFICIAL SHALL ENSURE
 THAT ANY PERSON CONDUCTING PLANS REVIEW IS QUALIFIED AS
 A PLANS EXAMINER UNDER PART XII OF CHAPTER 468, FLORIDA
 STATUTES, AND THAT ANY PERSON CONDUCTING INSPECTIONS IS
 QUALIFIED AS A BUILDING INSPECTOR UNDER PART III OF
 CHAPTER 468, FLORIDA STATUTES.
  
 3) A SAFETY PLAN SHALL BE SUBMITTED INDICATING THE
 CONSTRUCTION SAFEGUARDS TO BE UTILIZED AND SHALL MEET
 THE MINIMUM REQUIREMENTS OF THE FLORIDA EXISTING
 BUILDING CODE, CHAPTER 13. PLEASE SUBMIT SPECIFICATIONS
 FOR ANY WORK PLATFORMS THAT WILL BE USED.
  
 4) A "MAINTENANCE OF TRAFFIC" PERMIT IS REQUIRED WHEN
 ANY CITY RIGHT OF WAY IS PARTIALLY BLOCKED OR CLOSED.
  
 5) PLEASE PROVIDE NOTE INDICATING WHERE THE CUSTOM
 SHORING WILL TRANSFER LOADS.
  
 6) PLEASE INDICATE WELD DESIGNATIONS AT APPLICABLE
 CONNECTIONS.
  
 7) PLEASE PROVIDE THE MANUFACTURER'S SPECIFICATIONS FOR
 THE CONVENTIONAL SHORING.
  
 8) PLEASE PROVIDE CLARIFICATION: WILLTHECOLUMNS AT
 GRID LINE K-8,L-8,M-8 AND N-8 BE REPAIRED ON THE 3RD
 AND 4TH FLOOR?
  
 9) UPON REVIEWING THE BASIC LOAD FACTOR COMBINATIONS AS
 PER THE FBC, SECTION 1605 AND REVIEWING THE
 CALCULATIONS ON SHEET 6, THERE APPEARS TO BE NO WIND
 FACTOR WITHIN THE FORMER CALCULATION. PLEASE PROVIDE
 JUSTIFICATION.
  
 10) AS INDICATED ON THE CALCULATION SHEET #22, A W24X68
 TOP BEAM IS SHOWN TO BE ACCEPTED. HOWEVER, A W27X94 IS
 REFLECTED ON SHEET 3.0 OF THE SHORING PLAN.
