 AND AND ENERGY CALCULATIONS, CORRECT THE ADDRESS OF THE
 JOBSITE, MANY INDICATE WORK TO BE COMPLETED UNDER THIS
 PERMIT IS AT 4354 OKEECHOBEE BLVD.
  
 3) THE SHELL PERMIT IS A ISSUED PERMIT. WHEN THE
 BUILDING SHELL TEAM IS DIFFERENT FROM THE INTERIOR FIT
 UP , A SHOP DRAWING STAMP OF APPROVAL FROM THE BASE
 BUILDING DESIGN TEAM IS REQUIRED. 106.1.2 ADDITIONAL
 INFORMATION REQUIRED.
  
 4) THE BASE BUILDING CONTRACTOR WILL ALSO NEED TO
 PROVIDE A LETTER TO ALLOW THE TENANT FIT UP CONTRACTOR
 TO START WORK BEFORE THE ISSUANCE OF A CERTIFICATE OF
 COMPLETION THIS WILL BE REQUIRED TO BE NOTARIZED
 LETTER.

 SHEET BEARING THE NAME OF AN ENGINEER PRACTICING UNDER
 A FICTITIOUS NAME, A CORPORATION, OR A PARTNERSHIP,
 OFFERING ENGINEERING SERVICES, SHALL INCLUDE THE
 CERTIFICATE OF AUTHORIZATION NUMBER. ADD THE NUMBER TO
