 DENIED BY BUILDING
  
 1) A NOTICE OF COMMENCEMENT WILL BE REQUIRED.
  
 2) THE PBC PROPERTY APPRAISER LISTS THE VALUE OF THE
 STRUCTURE AS $ 20,552.00 AND THE VALUE OF THE WORK IS
 $100,000.00, WHICH IS MORE THAN FIFTY PER CENT OF THE
 VALUE OF THE STRUCTURE. THEREFORE, THE EXISTING
 RESIDENTIAL STRUCTURE IS REQUIRED TO HAVE THE LOWEST
 FLOOR ELEVATED TO SIX INCHES ABOVE BASE FLOOD
 ELEVATION. SEE ITEM #2 OF PREVIOUS REVIEW. SEE SECTION
 94-546 (B) (1) OF THE CITY CODE.
  
 3) SHOP DRAWINGS FOR THE SPIRAL STAIRS WILL BE REQUIRED
 TO BE SUBMITTED FOR PLAN REVIEW APPROVAL PRIOR TO
 INSTALLATION. FBC 106.1.1* CITY AMENDMENTS.
  
 4) SUBMIT PRODUCT APPROVALS/MANUFACTURER'S
 SPECIFICATIONS FOR THE SECOND AND THIRD FLOOR DECK
 WATERPROOFING. FAC RULE 9B-72 AND FBC 106.1.1* CITY
 AMENDMENTS.
  
 5)ON SHEET A-4 SEVERAL OF THE CONNECTIONS FOR THE
 BEAMS AND GIRDERS ARE NOT SPECIFIED. FBC 106.1.1* CITY
 AMENDMENTS.
  
 6) THE WALL SECTION ON A-5 SPECIFIES R-30 INSULATION IN
 A 4 INCH SPACE AT THE UPPER ROOF. PLEASE CLARIFY. SEE
 FBC 13-6.
  
 7)FBC 106.3* CITY AMENDMENTS PRODUCT APPROVALS.
 THOSE PRODUCT WHICH ARE REGULATED BY DCA RULE 9B-72
 SHALL BE REVIEWED AND APPROVED IN WRITING BY THE
 DESIGNER OF RECORD PRIOR TO SUBMITTAL FOR
 JURISDICTIONAL APPROVAL. THE HARDIPLANK PRODUCT
 APPROVALS HAVE NOT BEEN APPROVED BY THE DESIGNER OF
 RECORD.
  
 8) SUBMIT THE INSTALLATION INSTRUCTIONS/EVALUATION
 REPORT OR NOA FOR THE PGT EXTERIOR DOOR, SINGLE HUNG
 AND FIXED WINDOWS AS REQUIRED BY FAC RULE 9B-72.
  
 9) SPECIFY THE SIZE OF THE BASE PLATE FOR THE STEEL AND
 ALUMINUM COLUMNS. INDICATE THE LOCATIONS AND SIZE OF
 THE HOLES FOR THE ANCHOR BOLTS. FBC 106.1.2* CITY
 AMENDMENTS.
  
 10) THE HEIGHT OF THE BALCONY GUARDRAIL IS SPECIFIED AS
 BOTH 42" AND 36" ON SHEET A-3. PLEASE CORRECT.
