 PROPERTY THAT IS RENTED OR LEASED. THIS OWNER HAS THREE
 PROPERTIES ONE AT 616 - 48TH ST THAT IS CONSIDERED MAIN
 RESIDENCE AND HAS HOMESTEAD EXEMPTION. PLEASE FILL OUT
 A CHANGE OF CONTRACTOR FORM AND HAVE A LICENSED
 CONTRACTOR PERFORM THE WORK.
