 PROVIDE RE- RECORDED NOTICE OF COMMENCEMENT.
  
 3- PROVIDE COPY OF IMPACT FEES PAID RECEIPT AS

 REQUIRES THE FOLLOWING:
 (1) FLORIDA PRODUCT APPROVAL FL253 AND NOA
 05-0103.03 DON'T CORRESPOND TO EACH OTHER. THESE ARE
 TWO DIFFERENT PRODUCT APPROVALS. NEED TO SUBMIT
 COMPLETE INFORMATION FOR EITHER ONE.
  
 (2) FL253-R0DOESN'T HAVE ANY DRAWINGS AND IS FOR
 CODE VERSION 2001.NEED TO PROVIDE 2004 CODE VERSION.
 ARCHITECT TO MAKE SURE CORRECT CODE EDITION IS USED
 PRIOR TO APPROVAL.
  
 5- DESIGNER OF RECORD TO REVIEW AND APPROVE IN WRITING
 ALL PRODUCT APPROVALS AS REQUIRED BY SEC. 106.3.3 CITY
 AMENDMENTS TO FBC 2004. MAKE SURE NAME OR ARCHITECT OR
 COMPANY NAME IS INCLUDEDTO VERIFYTHAT DESIGNER OF
 RECORD IS THE ONE APPROVING THE PRODUCT APPROVALS.
  
 6- PROVIDE COMPLETE SPECS FOR BBQ GRILL AS REQUESTED ON
 PREVIOUS CHECKLIST.
 NOTE:MAKE SURE BBQ GRILL IS DESIGNED FOR INSTALLATION
 IN PORCH AS SPECIFIED ON PLANS. SEC. 106.1.2 CITY
 AMENDMENTS TO FBC 2004.
  
 7- ARCHITECT TO SPECIFY ROOF TILE FASTENING AS REQUIRED
 BY BY SEC. 106.1.1.2CITY AMENDMENTS TO FBC 2004. NEW
 DETAIL SUBMITTED ONLY IDENTIFIES RIDGE INSTALLATION,
 WHICH IS REQUIRED BY RAS 118.IT DOESN'T SPECIFY HOW
 ARE THE RESTOF TILES INSTALLED. IT'S GOING TO BE
 POLYPRO ADHESIVE OR NAILS OR SCREWS. IF USING NAILS OR
 SCREWS, THEN SPECIFY SIZE AND NUMBER. IF USING POLYPRO,
 THEN PROVIDE PRODUCT APPROVAL.
  
 8- SPECIFY ATTIC VENTILATION AT NEW CUPOLA AS REQUIRED
 BY SEC. R806 OF 2004 FLORIDA RESIDENTIAL CODE .EVEN
 THOUGH THE CUPOLA IS DECORATIVE, STILL VENTILATION IS
 REQUIRED. THERE ARE NOT EXCEPTIONS IN THE CODE.
  
 9- SPECIFY ALL LIVE AND DEAD LOADS, INCLUDING ATTIC
 LIVE LOADS AS REQUIRED BY TABLE R301.5 OF 2004 FLORIDA
 RESIDENTIAL CODE. ARCHITECT'S RESPONSE LETTER SAID "SEE
 REVISIONS TO SHEET S.3". THERE ARE NOT REVISIONS
 SHOWING THE LOADS ON SHEET S.3.
 IN ADDITION REVISE NOTE D4 ON SHEET S.1 TO INCLUDE ALL
 LOADS INCLUDING THE ATTIC LIVE LOAD.
  
 10- SOIL TEST REPORT PROVIDED REQUIRES THE FOLLOWING:
 A) ENGINEER'S ORIGINAL SIGNATURE AS REQUIRED BY
 CHAPTER 471 FLORIDA STATUTE AND SEC. 61G15-23.002 OF
 FLORIDA ADMINISTRATIVE CODE. REPORT SUBMITTED HAVE COPY
 OF THE SIGNATURE.
  
 B) REPORT NEEDS TO HAVE THE FIRM'S CERTIFICATE OF
