 3- BOTH COPIES OF SURVEYS NEED TO HAVE ORIGINAL SEAL,
 SIGNATURE AND DATE AS REQUIRED BY CHAPTER 472 FLORIDA
 STATUTES.
  
 4- PROVIDE FLORIDA OR LOCAL PRODUCT APPROVAL AS
 REQUIRED BY DCA RULE 9B-72 FOR:
 A) SINGLE HUNG WINDOWS.
 B) FIXED WINDOWS.
 C) ENTRY DOOR ASSEMBLY AS SHOWN ON PLANS AND
 ELEVATIONS.
 D) GARAGE DOOR.
 E) SLIDING GLASS DOORS.
 F) ALL SIMPSON AND USP CONNECTORS SPECIFIED ON PLANS
 AND DETAILS.
 G) PRECAST LINTELS.
 H) ROOF SHINGLES.
  
 5- ARCHITECT TO REVIEW AND APPROVE IN WRITTING ALL
 PRODUCT APPROVALS AS REQUIRED BY SEC. 106.3.3 CITY
 AMENDMENTS TO FBC 2004. INCLUDING THE STORM SHUTTERS.
  
 6-A) ARCHITECT TO PROVIDE ORIGINAL SIGNATURE IN ALL
 DRAWINGS AS REQUIRED BY CHAPTER 481 FLORIDA STATUTES
 AND CLARIFY WHY THERE ARE DRAWINGS WITH DIFFERENT
 SIGNATURES.
 B) ARCHITECT TO PROVIDE ALL INFORMATION REQUIRED BY
 FAC SEC. 61G1-16.004 IN THE TITLE BLOCK. SEE ATTACHED

 7- SURVEY AND ARCHITECT'S PLANS DON'T MATCH. REVISE AS
