 DENIED
  
 1) A RECORDED COPY OF THE NOTICE OF COMMENCEMENT
 MUST BE SUBMITTED BEFORE A
 PERMIT CAN BE ISSUED.
  
 2) IMPACT FEES MUST BE PAID TO PALM
 BEACH COUNTY, PLANS STAMPED BY THEM AND
 COPY OF RECEIPT SUBMITTED TO CITY OF
 WEST PALM BEACH BUILDING DEPARTMENT,
 BEFORE A BUILDING PERMIT CAN BE ISSUED.
  
 3)THE ENERGY CALCULATION FORMS THAT WERE SUBMITTED
 CAN ONLY BE USED FOR ADDITIONS OF 600 SQFT OR LESS.
 THIS ADDITION IS 952 SQFT.
 SUBMIT THE CORRECT FORMS. SEE FBC 13-6.
  
 4) PROVIDE A DRAINAGE PLAN FOR THE LOT AS REQUIRED BY
 FBC R401.3. THE SITE PLAN SUBMITTED DOES NOT ADEQUATELY
 INDICATE THE SURFACE WATER DRAINAGE REQUIREMENTS OF
 SECTION R401.3 OF THE 2004 FBC RESIDENTIAL CODE.
  
 5) THE LIVE LOAD FOR BALCONIES SHOULD BE 60 PSF UNDER
 STRUCTURAL NOTES ON SHEET D1. SEE TABLE R 301.5.
  
 6) SPECIFY THE REQUIRED ATTIC VENTILATION FOR THE
 SECOND FLOOR. SEE FBC R806.1. SPECIFY HOW THE ADDITIONS
 ATTIC WILL BE VENTED.
  
 7) SHOW COMPLIANCE WITH FBC R320.7 WHICH REQUIRES ALL
 BUILDINGS WITH EAVES LESS THAN 6"TO HAVE GUTTERS AND
 DOWNSPOUTS THAT DISCHARGE 1 FOOT FROM THE STRUCTURE.
  
 8) SPECIFY THE 2 LAYERS OF UNDERLAYMENT FOR THE 3:12
 PITCH CEMENT TILE ROOF THAT IS REQUIRED BY FBC
 R905.3.3.1 FOR LOW SLOPE ROOFS.
  
 9) PROVIDE A DETAIL FOR THE REQUIRED GUARD AT THE OPEN
 SIDES OF THE STAIRS, SHOWING COMPLIANCE WITH FBC
 R312.1, R312.2.2 AND TABLE R 301.5.
  
 10) PROVIDE HANDRAIL DETAILS AT THE STAIRS SHOWING
 COMPLIANCE WITH FBC R311.5.6, 311.5.6.1 AND 311.5.6.3.
  
 11) CITY OF WPB AMENDMENTS TO FBC 106.3.3 REQUIRES THE
 DESIGNER OF RECORD FOR THE PROJECT TO REVIEW AND
 APPROVE IN WRITING ALL OF THE PRODUCT APPROVALS. THE
 ENGINEER THAT SIGNED AND SEALED THE PLANS IS THE
 DESIGNER OF RECORD AND IS REQUIRED TO APPROVE THE
 PRODUCT APPROVALS.
  
 12) SPECIFYON THE PRODUCT APPROVAL NOA FOR THE MONIER
 SPANISH "S" CONCRETE TILE, WHICH ONE OF THE METHODS OF
 ATTACHMENTS LISTED WILL BE USED TO INSTALL THE TILE.
 THE FASTENERS THAT WERE SPECIFIED ON THE NOA DO NOT
 MEET THE REQUIRED MOMENT OF RESISTANCE.
  
 13) SPECIFY WHICH ONE OF THE APPROVED ASSEMBLIES AND
 DESIGN PRESSURE LISTED IN THE NOA FOR THE GAF BUR
 SYSTEM WILL BE INSTALLED. THE APPROVED ASSEMBLY AND
 MAXIMUM DESIGN PRESSURE WAS NOT SPECIFIED. SEE PAGES
 10-19 OF THE NOA. ENHANCED FASTENING MAY BE REQUIRED IN
 THE PERIMETER AND CORNER ZONES,SUBMIT THE RAS 117
 CALCULATIONS (SEE LIMITATION NUMBER 7).
  
 14) THE PRODUCT APPROVAL SUBMITTED FOR THE PGT SH-4000
 WINDOWS IS NOT FOR IMPACT RESISTANT WINDOWS AS
 SPECIFIED ON THE PLANS. IT IS ALSO EXPIRED.
  
 15) THE PRODUCT APPROVAL NOA FOR THE PGT HR710 IMPACT
 WINDOW IS EXPIRED. THE CURRENT ONE IS NOA 03-0612.06
 FOR FL 242-R3.
  
 16) SUBMIT PRODUCT APPROVALS FOR THE IMPACT RESISTANT
 FIXED GLASS WINDOW.
  
 17) SUBMIT PRODUCT APPROVALS FOR THE IMPACT RESISTANT
 STRUCTURAL WINDOW MULLIONS.
  
 18) IF THE NEW WINDOWS ARE NOT IMPACT RATED, THEN
 SPECIFY HOW THE GLAZED OPENINGS WILL BE PROTECTED FROM
 WINDBOURNE DEBRIS AS REQUIRED BY FBC R301.2.1.2. SUBMIT
 PRODUCT APPROVALS FOR STORM PANELS, WITH AN
 INSTALLATION SCHEDULE AND KEY PLAN.
  
 19) PR0VIDE A DETAIL INDICATING HOW THE DECK WILL BE
 WATERPROOFED SO THAT WHEN DECKING IS NAILED THROUGH THE
 ROOF MEMBRANE IT WILL NOT LEAK. ALSO SPECIFY THE
 FLASHING BETWEEN THE DECK AND THE EXTERIORWALL.
  
 20) FOUR SETS OF PLANS WERE SUBMITTED. TWO OF THE SETS
 DO NOT HAVE ALL SHEETS SIGNED AND SEALED. TWO COMPLETE
 SETS OF THE SIGNED AND SEALED DRAWINGS ARE REQUIRED.
 PLEASE REMOVE EXTRA PLANS TO AVOID CONFUSION. IF PLAN
 SHEETS ARE REVISED THEN YOU CAN SUBMIT ONE COPY OF THE
 OLD PLAN SHEETS FOR COMPARISON PURPOSES. HOWEVER,
 PLEASE MARK THE OLD SHEETS VOID OR OLD SHEETS.
  
 NOTE: MOST OF THE ABOVE COMMENTS ARE FROM THE FIRST
 REVIEW AND HAVE NOT BEEN ADDRESSED. IF YOU HAVE
