 ARE REQUIRED TO COMPLY WITH THE FLORIDA
 PRODUCT APPROVAL SYSTEM. FOR INFORMATION
 PLEASE SEE THE STATE WEBSITE AT
 WWW.FLORIDABUILDING.ORG. PRODUCTS WITH
 STATEWIDE APPROVAL ARE REQUIRED TO BE
 SUBMITTED WITH A COVER SHEET THAT LISTS
 THE PRODUCT IDENTITY NUMBER FROM THE
 STATE. IF THE PRODUCT DOES NOT HAVE
 STATEWIDE APPROVAL, SUBMIT AN APPLICA-
 TION FOR LOCAL PRODUCT APPROVAL OR SITE
 SPECIFIC FORM PER RULE 9B-72. THIS IS
 REQUIRED IN ADDITION TO THE NOA OR OTHER
 EVAUALATION DOCUMENTS.
  
 16) SUBMIT PRODUCT APPROVAL FOR STATE OR
 LOCAL WITH INSTALLATION INSTRUCTIONS FOR
 THE SLIDING GLASS DOORS.
  
 17) THE INSTALLATION DRAWINGS WITH THE
 KD WINDOWS NOA DO NOT HAVE THE MIAMI
 DADE NOA STAMP AND SIGNATURE ON THEM.
 THEY ARE NOT VALID WITHOUT THE STAMP AND
 SIGNATURE.
  
 18) SUBMIT TWO COPIES OF A KEY PLAN AND
 INSTALLATION SCHEDULE FOR THE STORM
 PANLES. LIST THE ANCHOR TYPE AND
 SPACING. SPECIFY THE MOUNT TYPE THAT
 WILL BE INSTALLED. A SEPARATE PERMIT
 WILL BE REQUIRED IF THIS INFORMATION IS
 NOT PROVIDED AT TIME OF PERMIT.
  
 19) THE INFORMATION ON THE ENERGY
 CALULATION FORMS DOES NOT MATCH THE
 PLANS. SEVERAL OTHER ITEMS ARE INCORRECT
 ON THE FORM.
  
 20) THE CALCULATED VALUE OF THE
 STRUCTURE HAS BEEN INCREASE BASED ON ICC
 CONSTRUCTION VALUE TABLES.
  
 PLEASE SUBMIT ONE COPY OF OLD PLAN
 SHEETS IF NEW SHEETS ARE SUBMITTED AS
 THIS WILL HELP TO FACILITATE THE PLAN
