 RECORD SHALL HAVE THE SIGNATURE AND
 SEAL OF THE DESIGN PROFESSIONAL AFFIXED

 PROFESSIONAL. WHEN PBSJ, POST,BUCKLEY,
 SCHUH & JERNIGAN INC NAMES APPEAR ON THE
 HEADER STRIP THIS REQUIRES SEALED PLANS.
 1B(2) IF THE NAMES MENTIONED ABOVE ARE
 NOT ON THE HEADER STRIP WE DO NOT THEN
 HAVE TO MEET THIS OBLIGATION, BUT UNDER
 FBC 104.2.1*ALL INFORMATION,
 DRAWINGS, SPECIFICATIONS & ACCOMPANYING
 DATA SHALL BEAR THE NAME AND SIGNATURE
 OF THE PERSON RESPONSIBLE FOR THE
 DESIGN. NOTE!!!! I AM NOT REQUESTING YOU
 TO SIDE STEP COPYRIGHT LAWS BUT IF
 SOMEONE DOES THIS, THEY THEN HAVE TO

 PACKAGES, REMOVE THE EXCESS PAPER NOT
 REQUIRED FOR REVIEW. EXAMPLETHERE IS
 SITE PLANS WITH PBSJ AND SITE PLANS
 WITHOUT THIS HEADING BUT NO ONE TAKES
 RESPONSIBLITY FOR THE DRAWING? MAKE (2)
 SETS OF PLANS TO BE REVIEWED!!!!!
  
 1D) THERE ARE ENGINEERED PLANS FROM
 GLEN TISDALE (2) SETS EACH DESIGNED
 UNDER THE 2001 FL BLDG CODE AND (2) SETS
  
 UNDER THE 2004 FL BLDG CODE, REMOVE THE
 2004 SINCE THIS PERMIT WAS FILED UNDER
 THE 2001 FL BLDG CODE.
  
 2) THE NOTICE OF COMMENCEMENT HAS NEVER
 BEEN FILED AT THE CLERK OF THE COURT.
 THE COUNTY STAMP FROM THE CLERK OF THE
 COURT MUST BE ON THE NOC AND THE DATE
 RECORDED, WORK MUST START WITHIN 90 DAYS
 OF THE RECORDED DATE OR THE NOC MUST BE
 REFILED. FL S S 713.13.
  
 3) PREVIOUSLY CONTRACTOR HAD GIVEN US
 THE VALUE OF THE PROJECT AS $16,000.00
 DOLLARS SEE ATTACHMENT FOR BALANCE DUE.
  
 4) REQUIRES THE ORIGINAL PERMIT
 APPLICATION OR A NEW APPLICATION WITH
 SIGNATURES SHALL BE REQUIRED.
  
 WHEN RESUBMITTING PLANS PLEASE INDICATE
 THE REVISION & REMOVE & REPLACE ANY
 PAGES AS NECESSARY. A TRANSMITTAL LETTER
 LISTING THE ORIGINAL REVIEW COMMENT NUM-
 BER, WITH A DESCRIPTION OF THE REVISION
 MADE, IDENTIFYING THE SHEET OR SPECIFICA
 TION PAGE WHERE THE CHANGES CAN BE FOUND
 WILL HELP TO EXPEDITE YOUR PERMIT. THANK
