 $56,027.00.ADDITIONAL PERMIT FEES OF

 APPROVALS WITH QUALITY ASSURANCE FOR THE
 FOLLOWING.ROOFING, WINDOWS, EXTERIOR
 DOORS, IMPACT PROTECTION, STRAPS AND
 TIE-DOWNS AND LINTELS.
 ALL PRODUCT APPROVALS SUBMITTED SHALL
 HAVE THE FOLLOWING ATTACHED.

 PERMIT APPLICATION AFTER OCTOBER 1, 2003
 ARE REQUIRED TO COMPLY WITH THE FLORIDA
 PRODUCT APPROVAL SYSTEM. FOR INFORMATION

 ALLOWED.IF THE GRADE ISN'T EXACTLY
 CORRECT EITHER THE FOOTING WON'T BE DEEP
 ENOUGH OR THE GRADE WILL BE TOO HIGH.

 THE REVISION & REMOVE & REPLACE ANY
 PAGES AS NECESSARY. A TRANSMITTAL LETTER
 LISTING THE ORIGINAL REVIEW COMMENT NUM-
 BER, WITH A DESCRIPTION OF THE REVISION
 MADE, IDENTIFYING THE SHEET OR SPECIFICA
 TION PAGE WHERE THE CHANGES CAN BE FOUND
