 1.)PROVIDE FIRE RATING INFORMATION
 FOR THE ENTRY DOOR.THIS DOOR IS
 REQUIRED TO BE A B-LABEL DOOR
 AS PER CBAA CASE NUMBER 1224 (SEE
 ATTACHED), WITH A DOOR CLOSER AS PER
 KEYNOTE 6, SHEET A-T3.1.
  
 2.)THE ARCHITECT OR ENGINEER OF
 RECORD IS TO REVIEW AND APPROVE THE
 PRODUCT APPROVALS.
  
 3.)A ROOF PERMIT IS REQUIRED FOR THE
 ROOFING WORK.EITHER HAVE THE ROOFING
 CONTRACTOR SIGN THE BACK OF THE
 APPLICATION, OR SUBMIT THE ROOF PRODUCT
 APPROVALS SEPARATELY WITH THE ROOF
 PERMIT APPLICATION.
  
 5.)TWO COPIES OF EACH PRODUCT APPROVAL
 ARE REQUIRED PER PERMIT (THREE WERE
 SUBMITTED).
  
 7.)SUBMIT PRODUCT APPROVALS FOR THE
 LOUVERED EQUIPMENT ROOM DOORS.THESE
 DOORS ARE REQUIRED TO BE 3/4 HOUR RATED
 FIRE DOORS; SUBMIT PRODUCT INFORMATION.
  
 8.)PRODUCT APPROVALS FOR A GARAGE DOOR
 ARE NOT NECESSARY AS THERE AREN'T ANY
 GARAGE DOORS ON THE BUILDING.
  
 9.)COMPLETE FILLING OUT THE PERMIT
 APPLICATIONS.REMOVE THE WORK FROM
 THE DESCRIPTION WHICH WILL NOT BE
 PERFORMED UNDER THIS PERMIT (ROOF IF
 BEING APPLIED FOR SEPARATELY, GARAGE
 DOORS)."MASTER PERMIT NUMBER" SHOULD
 BE THE BUILDING PERMIT NUMBER FOR THAT
 ADDRESS.INCLUDE A VALUE.
  
 10.)THERE IS A CONFLICT ON THE PLAN
 BETWEEN SHEETS A-T3.1 AND A-T3.2 AND
 CBAA CASE #1224.THE WALL, KEYNOTE
 1 ON SHEET A-T3.2, REFERS TO: 5"
 CONCRETE WALL", BUT THE FLOOR PLAN DOES
 NOT IDENTIFY THE CORRECT WALL.
  
 11.)THERE IS A CONFLICT ON THE PLAN
 BETWEEN KEYNOTE, SHEET A-T3.2, AND THE
 CBAA CASE #1224.KEYNOTE 3 REFERS TO:
 5" CONC. WALL - 1 HR FIRE RATED.THIS
 SHOULD BE IDENTIFIED AND DESIGNED AS
 A 2 HOUR FIRE RATED WALL.
  
 12.)THE ENTRANCE DOORS TO THE UNITS,
 IDENTIFIED ON SHEET A-T3.1, KEYNOTE 6,
 ARE REFERRED TO AS A "C" LABEL 1 HR
 FIRE RATED DOOR.THIS SHOULD BE A
 B LABEL DOOR AS PER CBAA CASE NO. 1224.
  
 13.)SHEET A-T3.2, KEYNOTE 4, TEXT
 STATES IS A 6" EXT. METAL STUD EXTERIOR
 WALL.THE TENANT SEPARATION IS
 INCORRECTLY NOTED AS SUCH.
  
 14.)REGARDING THE ABOVE CORRECTIONS
 #10 THROUGH #13, THE DESIGNER OF RECORD
 IS TO MAKE AN APPOINTMENT TO CHANGE THE
 PLANS FOR ALL PERMITS AFFECTED.CALL
