 1) THE LETTER FROM PALM BEACH LIFTS IS
 REQUIRED TO HAVE AN ORIGINAL SIGNATURE
 AND BE NOTARIZED.
  
 2) THE STORM PANEL INSTALLATION LETTER
 IS REQUIRED TO BE ADDRESSED TO THE
 BUILDING OFFICIAL (NEIL MELICK). IT IS
 REQUIRED TO BE FROM A PRINCIPAL OF THE
 COMPANY. IT MUST BE SIGNED AND NOTARIZED
 OR HAVE THE CORPORATE SEAL.
  
 3) THE COVER LETTER INDICATES THAT THE
 SIDELITES AT THE ENTRY DOORS FOR BOTH
 THE FIRST AND SECOND FLOOR UNITS ARE
 REQUIRED TO HAVE STORM PANELS. THE PRE-
 STORM PLAN AND THE CONDO DOCUMENTS DO
 NOT INDICATE THIS. THEY BOTH STATE THAT
 THE SECOND FLOOR UNITS HAVE IMPACT
 WINDOWS AND DOORS AND DO NOT REQUIRE
 STORM PANELS. THE SIDELIGHT OF THE ENTRY
 DOORS TO THE SECOND FLOOR UNITS THAT ARE
 REQUIRED TO BE SHUTTERED MUST BE
 ADDRESSED. THE LETTER, THE PREPARATION
 PLAN AND THE CONDO DOCUMENTS MUST ALL BE
 IN AGREEMENT.
  
 4) THE ENERGY CALCULATION FORM FOR UNIT
 "D" IS NOT INDICATED AS WORST CASE. THIS
 WILL REQUIRE THE FORM FOR THIS ONE UNIT
 TO HAVE THE SITE LOCATION ON THE FORM &
 TO BE SUBMITTED WITH EACH BUILDING
 PERMIT APPLICATION.
 THE PERSON PREPARING THE ENERGY CALCS.
 ARE REQUIRED TO SIGN AND DATE THEM.
  
 5) A SIGNED AND SEALED COPY OF THE SOIL
 REPORT IS REQUIRED.
