 1) PROVIDE A SURVEY, PREPARED BY A FLORIDA LICENSED
 SURVEYOR, THAT SHOWS THE RIGHT-OF-WAY LINE, THE
 SIDEWALK, AND ANY TREES WITHIN THE RIGHT-OF-WAY ALONG
 THE FIBER CONDUIT PATH SO THAT STAFF CAN VERIFY THAT
 THE BORE AND ANY REQUIRED BORE PITS AND HANDHOLES ARE
 WITHIN THE RIGHT-OF-WAY. THE SURVEY NEEDS TO INCLUDE
 THE LOCATION OF PROPOSED HANDHOLES 00+00, 01+10 AND THE
 BORE SECTION ALONG THE WEST SIDE OF SOUTH DIXIE HIGHWAY
 SINCE A SURVEY THAT THE CITY HAS ON FILE SHOWS THAT THE
 AREA BETWEEN THE FENCE AND THE SIDEWALK AS PRIVATE
 PROPERTY, NOT RIGHT-OF-WAY. IN ORDER TO INSTALL
 HANDHOLES AND BORE UNDER PRIVATE PROPERTY IS MUST BE IN
 A GENERAL UTILITY EASEMENT. THIS GENERAL UTILITY
 EASEMENT MUST BE CLEARLY SHOWN ON THE SURVEY.
  
 2) THE TUBULAR MARKERS ARE NOT PERMITTED. MARKERS MAY
 ONLY BE INSTALLED WITHIN THE HANDHOLES OR OTHERWISE
 BURIED.
  
 3) THE PLANS MUST SHOW THE DIMENSION BETWEEN THE PALM
 TREE AND THE EXCAVATION FOR HANDHOLE 04+55 OF AT LEAST
 THREE FEET FROM THE TRUNK OF THE PALM TREE.
 ADDITIONALLY, TO PROTECT THE PALM TREE TRUNK FROM
 DAMAGE, THE BORE MUST COME FROM THE WEST. THE BORE
 MACHINE MAY NOT STAGE ADJACENT TO THE PALM TREE.
  
 4) THE BORE DEPTH IS SUFFICIENT TO AVOID TREE ROOTS.
