 1) THIS LOCATION IS WITHIN A HISTORIC DISTRICT. PLEASE
 PROVIDE A COPY OF THE STATE HISTORIC PRESERVATION
 OFFICE APPROVAL OF THIS LOCATION.
  
 2) THE SURVEY AND ENGINEERING PLANS REFERENCE A WOOD
 DISTRIBUTION POLE TO ESTABLISH THE HEIGHT OF A NEARBY
 POLE TO JUSTIFY THE HEIGHT OF THE PROPOSED WIRELESS
 FACILITY POLE; HOWEVER, THE GOOGLE STREET VIEW SHOWS
 THE POLE AS CONCRETE. THE CORRECT INFORMATION MUST BE
 PROVIDED ON THE SURVEY AND THE PLANS SO THAT STAFF CAN
 VERIFY COMPLIANCE WITH THE CODE REQUIREMENTS.
  
 3) THE PLANS DO NOT CLEARLY SHOW THE POLE BASE PLATE
 NOT ENCROACHING ON THE SIDEWALK. PLEASE PROVIDE
 VERIFICATION THAT THE EDGE OF THE BASE PLATE WILL NOT
 OVERLAP THE SIDEWALK.
  
 4) THE BORE PIT FOR THE HANDHOLE BY THE POLE NOTES ON
 SHEET P-1 STATE THAT THE SIDEWALK WILL BE RESTORED;
 HOWEVER, THE PARKWAY WHICH WILL NEED RESTORATION.
 PLEASE UPDATE THE NOTATIONS ACCORDINGLY.
  
 5) PROVIDE A CROSS SECTION OF THE UNDERGROUND ELECTRIC
 WHICH VERIFIES THAT THERE ARE NO CONFLICTS WITH
 EXISTING UTILITIES.
  
 6) THE POLE STYLE AND HEIGHT MEET CITY REQUIREMENTS.
