 1) PROVIDE A SURVEY, PREPARED BY A FLORIDA LICENSED
 SURVEYOR, WHICH SHOWS THE POLE LOCATION AND THE
 RIGHT-OF-WAY LINE SO THAT STAFF CAN VERIFY THAT THE
 WIRELESS COMMUNICATION FACILITY IS WITHIN THE
 RIGHT-OF-WAY AND NOT ON PRIVATE PROPERTY. THE SURVEY
 MUST SHOW THE SETBACKS OF THE PROPOSED POST TO THE EDGE
 OF THE RIGHT-OF-WAY AND THE EDGE OF THE SIDEWALK, AS
 WELL AS THE SIDEWALK WIDTH. STAFF HAS NOTED IN A REVIEW
 OF THE PROPOSED LOCATION ON GOOGLE STREETVIEW THAT
 THERE ARE UTILITY POLES AND PARKING METERS IN THE AREA.
 THESE SHALL BE SHOWN ON THE SURVEY WITH THE DIMENSIONS
 FROM THE CLOSEST FEATRUES TO THE PROPOSED POLE, TO
 INCLUDE THE GUY WIRES. ADA CLEARANCES WILL NEED TO BE
 CLEARLY SHOWN AS MET.
  
 2) THE SURVEY MUST INCLUDE THE HEIGHTS OF UTILITY POLES
 (INCLUDING LIGHT POLES) ALONG 4TH STREET, SO THAT STAFF
 CAN VERIFY THAT THE PROPOSED 25 FOOT TALL POLE COMPLIES
 WITH THE POLE HEIGHTS LOCATED ALONG 4TH STREET.
  
 3) THE POLE COLOR WILL NEED TO BE BLACK TO MATCH THAT
 DECORATIVE STREET LIGHTS THAT WILL BE PLACED ON THE
 NORTH SIDE OF 4TH STREET.
  
 4) THE PROPOSED POLE COMPLIES WITH CITY CONCEALMENT
 REQUIREMENTS.
