 1) SHEET T1 LISTS THE SCOPE OF WORK INCLUDES THE
 INSTALLATION OF AN ENCLOSURE CABINET. NONE OF THE PLANS
 SHOW AN ENCLOSURE CABINET. PLEASE CLARIFY WHAT THIS
 MEANS. THE CITY DOES NOT PERMIT FREESTANDING OR POLE
 MOUNTED CABINETS.
  
 2) THE UTILITY POWER PLAN WAS NOT INCLUDED (THIS IS
 SHEET E2 ON THE ELECTRICAL PERMIT 20101235 SUBMITTAL)
 THAT SHOWS THE UNDERGROUND POWER THAT IS BEING PULLED
 FROM 4TH STREET. THE UNDERGROUND POWER PATH, INCLUDING
 ANY BORE PITS AND HANDHOLES ARE REQUIRED TO BE SHOWN ON
 THE WIRELESS COMMUNICATION FACILITY ROW PERMIT
 APPLICATION.
  
 3) PROVIDE A SURVEY, PREPARED BY A FLORIDA LICENSED
 SURVEYOR, SHOWING THE RIGHT-OF-WAY LINE AND THE EDGE OF
 THE SIDEWALK, WITH DIMENSIONS FOR THE UNDERGROUND POWER
 FROM THE WIRELESS POLE ON 3RD STREET TO THE ELECTRIC
 SERVICE POLE ON 4TH STREET.
  
 4) THE ENGINEERING PLAN (UTILITY POWER PLAN) NEEDS TO
 SHOW THE RIGHT-OF-WAY LINE (BASED ON THE SURVEY) AND
 CLEARLY DEMONSTRATE THAT THE HANDHOLES, BORE PITS AND
 UNDERGROUND POWER BORE ARE COMPLETELY LOCATED WITHIN
 THE RIGHT-OF-WAY. NOTE: THE BORE APPEARS TO CROSS ONTO
 PRIVATE PROPERTY ON THE UTILITY POWER PLAN INCLUDED
 WITH PERMIT 20101235. IF THE SURVEY SHOWS A GENERAL
 UTILITY EASEMENT AT THIS LOCATION, THEN THE UNDERGROUND
 FEATURES MAY CROSS INTO THE EASEMENT.
  
 THE POLE LOCATION AND STEALTH DESIGN MEETS CITY
 STANDARDS.
