 1). UTILITY RELEASE - CITY OF WEST PALM BEACH WATER
 DEPARTMENT,, ON THEIR FORM "DEMOLITION APPLICATION"

 IF THE FORM IS NOT AVAILABLE ONLINE, SEND A REQUEST FOR

 IF A WATER TRUCK IS DESIRED FOR WATERING DOWN SITE,
 DETAILS WILL BE REQUIRED.
  
 IF A HYDRANT METER OR BACKFLOW PREVENTER IS DESIRED,
 THEN A PARTIAL RELEASE WILL BE NOTED ON THEIR FORM.
 IF A BACKFLOW PREVENTER DEVICE HAS BEEN SELECTED FOR
 THE WATER DOWN METHOD AND A PERMIT HAS BEEN OBTAINED -
 BEFORE SCHEDULING THE FINAL INSPECTION, THE BACKFLOW
 DEVICE NEEDS TO BE TESTED AND CERTIFIED BY THE CITY'S
 UTILITIES DEPT, 561-822-2244.
  
 FAX THE COMPLETED "DEMOLITION APPLICATION" FORM TO
