 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) SURVEY - NO SURVEY WAS PROVIDED.
  
 A. THE SURVEY, PREPARED BY A FLORIDA REGISTERED
 SURVEYOR, MUST CLEARLY SHOW THE RIGHT-OF-WAY LINE, THE
 SIDEWALK AND THE UTILITY EASEMENT LISTED ON THE
 ENGINEERING PLANS.
  
 B. ALL UNDERGROUND UTILITIES NEED TO BE PROVIDED ON THE
 SURVEY. IF THE BORE IS PROPOSED WITHIN FIVE (5) FEET OF
 AN EXISTING UTILITY, YOU NEED TO PROVIDE A LETTER FROM
 THAT UTILITY THAT THE PROPOSED BORE DOES NOT INTERFERE
 WITH THE PROVISIONS OF THEIR SERVICES.
  
 C. THE TREES AND THEIR ROOT ZONE NEED TO BE SHOWN. THE
 FIBER BORE NEEDS TO BE CLEARLY SHOWN AVOIDING DAMAGE TO
 THE TREE ROOT ZONES.
  
 D. THE ENGINEERING PLANS MUST SHOW THE SAME CONDITIONS
 AS THE SURVEY, SO THAT STAFF CAN VERIFY THAT NONE OF
 THE PROPOSED WORK IS BEING INSTALLED ON PRIVATE
 PROPERTY. NOTE: THE ENGINEERING PLANS MUST BE AT A
 10FT, 20FT, OR 40FT SCALE, NOT THE 50FT SCALE THAT WAS
 PROVIDED.
  
 2) THE 900 BLOCK OF BANYAN BOULEVARD RIGHT-OF-WAY IS
 UNDER CONSTRUCTION. WHILE THE ENGINEERING PLANS THAT
 YOU SUBMITTED SHOW SOME OF THE PROPOSED CONSTRUCTION,
 THEY NEED TO CLEARLY INDICATE THE LIMITS OF THE SOIL
 CELLS AROUND THE TREES, SO THAT STAFF CAN VERIFY THAT
 THE PROPOSED HANDHOLE STA 15+65 AND STA 19+20 ARE
 SUFFICIENTLY DISTANCED SO THAT WHEN THEY ARE INSTALLED
 THEY DO NOT DAMAGE THE SOIL CELL INFRASTRUCTURE. NOTE:
 DUE TO THIS BEING AN OPEN CITY CONSTRUCTION PROJECT,
 YOU MAY NOT BE ABLE TO INSTALL THESE HANDHOLES AND
 COMPLETE THE BORE UNTIL THE PROJECT IS COMPLETED.
  
 3) THE NEW HANDHOLE AT STA 8+50 AND THE EXCAVATION TO
 INSTALL IT IS TOO CLOSE TO AN EXISTING TREE AND ITS
 UNDERGROUND SOIL CELL. YOU MUST CLEARLY INDICATE ON THE
 ENGINEERING PLAN THAT YOU WILL BE MAINTAINING AT LEAST
 A SIX (6) FOOT SEPARATION FROM THE EDGE OF THE TREE
 GRATE TO THE EDGE OF ANY EXCAVATION TO AVOID DAMAGING
 THE SOIL CELL SYSTEM.
  
 4) INVENTORY OF REGISTRANT'S EXISTING FACILITIES -
 OTHER THAN THE MENTION OF TWO EXISTING HANDHOLES, NO
 INFORMATION ON EXISTING CROWN CASTLE FACILITIES IN THE
 AREA WAS PROVIDED. THIS MAY EITHER BE ON THE PLANS OR A
 SEPARATIVE NARRATIVE. IF THERE ARE NO ADDITIONAL
 FACILITIES, THIS SHALL BE STATED.
  
 5) ESTIMATED SCHEDULE - NO SCHEDULE WAS PROVIDED.
 PROVIDE A NARRATIVE OF THE ESTIMATED SCHEDULE OF THE
 PROPOSED WORK.
  
 6) WHEN RESUBMITTING THE PLANS, INCLUDE THE PERMIT
 NUMBER (21010042) AND A WRITTEN RESPONSE TO THE
 COMMENTS.
  
 7) THE BORE SHOWN AT 10 TO 14 FEET BELOW GRADE IS AT A
 SUFFICIENT DEPTH TO AVOID TREE AND PALM DAMAGE, SUBJECT
 TO ADDRESSING COMMENT #3.
  
 8) THE INSTALLATION CONTRACTOR MUST CONTACT SUNSHINE
 811 TO VERIFY THE LOCATION OF ALL UTILITIES AND VERIFY
 ALL DEPTHS PRIOR TO CONSTRUCTION. ALL CITY UTILITIES
 MUST BE LOCATED BY SOFT DIG METHODS BEFORE STARTING THE
 WORK.
  
 9) THE RIGHT-OF-WAY USE PERMIT REQUIRES YOU TO PROVIDE
 A CERTIFICATE OF LIABILITY INSURANCE, WHICH IN
 ACCORDANCE WITH SECTION 78-406 OF THE CITY CODE
 SPECIFICALLY NAMES THE CITY OF WEST PALM BEACH, ITS
 COMMISSIONERS, OFFICERS, EMPLOYEES AND AGENTS AS
 ADDITIONAL INSURED.
  
 10) MARKER POLES MAY NOT BE INSTALLED WITHIN THE CITY
 RIGHT-OF-WAY OR ON PRIVATE PROPERTY.
  
 11) THE FUTURE WIRELESS FACILITY POLE INSTALLATION WILL
 REQUIRE THE APPROVAL OF A WIRELESS COMMUNICATION
 FACILITY IN THE RIGHT-OF-WAY PERMIT.
