2020-08-11 13:47:571. PER PUBLIC UTILITIES AND ENGINEERING, THERE IS A 12
 INCH WATER MAIN ALONG THE WEST SIDE OF S FLAGLER RIGHT
 OF WAY. ONCE THE ACTUAL LINE IS LOCATED, THERE IS A
 REQUIREMENT FOR ROOT BARRIER 5 FEET FROM THE PIPE
 BEFORE ANY TREES OR PALMS COULD BE PLANTED. THIS DOES
 NOT APPEAR POSSIBLE WITH THE SPACE AVAILABLE BETWEEN
 THE CURB AND SIDEWALK.
  
 2. THERE ARE FOUR EXISTING BLACK OLIVE STREET TREES IN
 THE PARKWAY ON FOREST HILL BLVD. IF THE INTENT IS TO
 REMOVE THEM, SHOW DIAMETER OF EACH TREE CANOPY ON THE
 PLAN AND CALCULATE THE CANOPY SQUARE FOOTAGE. SHOW
 REPLACEMENT CATEGORY 1 SHADE TREES CONSISTENT WITH THE
 OTHER SHADE TREES ON THE STREET.
  
 3. PROVIDE A SEPARATE PLANT LIST FOR THE RIGHT OF WAY
 LANDSCAPING.
  
 4. SHOW THE SAFE SIGHT TRIANGLE ON THE PLAN WITHIN THE
 PROPERTY LINE AND CONTINUE THE LINES OUT INTO THE RIGHT
 OF WAY TO THE CURB. SECTION 94-305 (E) VISIBILITY AT
 INTERSECTIONS APPLIES.
  
 5. SUPPLY DETAILS FOR THE PAVED AREAS SHOWN ON THE PLAN
 BETWEEN THE SIDEWALK AND THE CURB, INCLUDE MATERIALS
 AND CONSTRUCTION. THIS MAY NEED TO BE SUBMITTED AS A
 REVISION TO THE DRIVEWAY PERMIT FOR RIGHT OF WAY
 CONSTRUCTION.
  
 6. ANY LANDSCAPING BESIDES GROUNDCOVER MUST BE
 MAINTAINED BY THE ADJACENT PROPERTY OWNER AND WILL
 REQUIRE A SIGNED MAINTENANCE AGREEMENT. THIS IS A ONE
 PAGE SIMPLIFIED FORM.
