 CITING SECTION 502.14 EXCEPTION #3. A) THE EOR MUST
 PROVIDE THESE STATEMENTS FROM THE NARRATIVE ONTO THE
 PLAN. B) THE EOR AND/OR CONTRACTOR SHALL PROVIDE A
