 PROVIDE WRITTEN RESPONSE TO COMMENTS.
  
 THE FOLLOWING ITEMS NEED TO BE ADDRESSED OR THE PERMIT
 WILL NOT BE APPROVED.
  
 1) THE MAJORITY OF THE FIBER RUN IS SHOWN ON PRIVATE
 PROPERTY WITHIN THE UTILITY EASEMENT. WIRELESS
 COMMUNICATION FACILITY WORK ON PRIVATE PROPERTY WITHIN
 A UTILITY EASEMENT IS NOT SUBJECT TO A WIRELESS
 COMMUNICATION FACILITIES IN A RIGHT-OF-WAY PERMIT. THE
 PERMIT WILL APPLY TO THE SECTION WHERE THE BORE CROSSES
 NORTHPOINT PARKWAY. THE MOT AND A RIGHT-OF-WAY PERMIT
 FOR ANY USE OF THE RIGHTS-OF-WAY WILL STILL BE REQUIRED
 FOR THE WORK WITHIN THE EASEMENT.
  
 2) ENGINEERING PLAN -
  
 A. REGARDLESS OF THE STATEMENT IN COMMENT 1, ABOVE,
 PLEASE PROVIDE VERIFICATION THAT THE PROPOSED TIE IN AT
 11+75 WILL NOT DAMAGE THE ROOT ZONE OF THE ADJACENT
 TREES.
  
 B. THE BORE SHOWN AT 12 FEET BELOW GRADE IS AT A
 SUFFICIENT DEPTH TO AVOID TREE AND PALM DAMAGE.
  
 C. STAFF WILL NOT APPROVE THE INSTALLATION OF A SIX (6)
 FOOT TALL MARKER POLE ADJACENT TO THE HANDHOLES (SHOWN
 ON SHEET 8). STAFF HAS REVIEWED A NUMBER OF MCIMETRO
 APPLICATIONS THAT COMPLY WITH THIS REQUIREMENT, SO THIS
 APPLICATION NEEDS TO SHOW THE SAME.
  
 PLEASE NOTE:
  
 THE INSTALLATION CONTRACTOR MUST CONTACT SUNSHINE 811
 TO VERIFY THE LOCATION OF ALL UTILITIES AND VERIFY ALL
 DEPTHS PRIOR TO CONSTRUCTION. ALL CITY UTILITIES MUST
 BE LOCATED BY SOFT DIG METHODS BEFORE STARTING THE
 WORK.
  
 THE RIGHT-OF-WAY USE PERMIT REQUIRES YOU TO PROVIDE A
 CERTIFICATE OF LIABILITY INSURANCE, WHICH IN ACCORDANCE
 WITH SECTION 78-406 OF THE CITY CODE SPECIFICALLY NAMES
 THE CITY OF WEST PALM BEACH, ITS COMMISSIONERS,
 OFFICERS, EMPLOYEES AND AGENTS AS ADDITIONAL INSURED.

 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) SURVEY - NO SURVEY WAS PROVIDED.
  
 A. THE SURVEY, PREPARED BY A FLORIDA REGISTERED
 SURVEYOR, MUST CLEARLY SHOW THE RIGHT-OF-WAY LINE, THE
 SIDEWALK AND THE UTILITY EASEMENT LISTED ON THE
 ENGINEERING PLANS.
  
 B. ALL UNDERGROUND UTILITIES NEED TO BE PROVIDED ON THE
 SURVEY. IF THE BORE IS PROPOSED WITHIN FIVE (5) FEET OF
 AN EXISTING UTILITY, YOU NEED TO PROVIDE A LETTER FROM
 THAT UTILITY THAT THE PROPOSED BORE DOES NOT INTERFERE
 WITH THE PROVISIONS OF THEIR SERVICES.
  
 C. THE TREES AND THEIR ROOT ZONE NEED TO BE SHOWN. THE
 FIBER BORE NEEDS TO BE CLEARLY SHOWN AVOIDING DAMAGE TO
 THE TREE ROOT ZONES.
  
 D. THE ENGINEERING PLANS MUST SHOW THE SAME CONDITIONS
 AS THE SURVEY, SO THAT STAFF CAN VERIFY THAT NONE OF
 THE PROPOSED WORK IS BEING INSTALLED ON PRIVATE
 PROPERTY.
  
 E. PLEASE NOTE THAT ANY WORK BEING DONE ON PRIVATE
 PROPERTY REQUIRES A SEPARATE APPLICATION AND REVIEW
 PROCESS AND IS NOT REVIEWED AS PART OF THE WIRELESS
 COMMUNICATION FACILITIES IN A RIGHT-OF-WAY.
  
 3) TIMETABLE - YOU ARE REQUIRED TO PROVIDE THE
 ESTIMATED TIMETABLE FOR CONSTRUCTION OF THE PROJECT OR
 EACH PHASE THEREOF.
  
 4) EVIDENCE OF INSURANCE COVERAGE - PROVIDE EVIDENCE OF
 THE INSURANCE COVERAGE REQUIRED UNDER SECTION 78-406 OF
 THE CITY CODE.
  
 5) POLE STYLE - PLEASE NOTE THAT THE FUTURE POLES ARE
 REQUIRED TO MEET THE CITY CONCEALMENT STANDARDS
 OUTLINED IN SECTION 78-411 OF THE CITY CODE.
  
 NOTE: STAFF WILL NOT APPROVE THE INSTALLATION OF A SIX
 (6) FOOT TALL MARKER POLE ADJACENT TO THE HANDHOLES. IF
 THE HANDHOLES NEED TO BE MARKED, THEN PROVIDE THE
 INFORMATION ON THE HANDHOLE COVER.
  
 6) ELECTRONIC FORMAT OF SUBMITTAL - A COPY OF ALL OF
 THE SUBMITTAL MATERIAL MUST BE SUBMITTED IN ELECTRONIC
