 BASE PLATE. THE BASE PLATE SETS THE WIDTH OF THE ABOVE
 GROUND FACILITY AT GRADE. SHEET C-3 SHOWS THE CAISSON
 EXTENDING 8-INCHES PAST THE EDGE OF THE POLE, WHICH IS
 7-INCHES PAST THE EDGE OF THE BASE PLATE.
  
 B. THE SURVEY MUST CLEARLY SHOW THE DIMENSION FROM THE
 INSIDE EDGE OF THE CURB TO THE RIGHT-OF-WAY LINE AND
 THE DIMENSION FROM THE INSIDE EDGE OF THE CURB FOR THE
 WIDTH OF THE SIDEWALK. THE 6-INCH CURB MAY NOT BE
 INCLUDED IN THIS MEASURE, SINCE THE 6-INCH CURB IS NOT
 CONSIDERED A FLUSH SURFACE AND MAY NOT BE USED TO MEET
 THE REQUIRED ADA MINIMUM SIDEWALK WIDTH OF 36-INCHES.
  
 C. BASED ON THE INFORMATION THAT IS REQUIRED ON THE
 SURVEY AND THE POLE SPECIFICATIONS, STAFF WILL VERIFY
 THAT THERE IS ADA SIDEWALK CLEARANCE. NOTE: IF THE
 CAISSON IS INSTALLED AT THE EDGE OF THE RIGHT-OF-WAY,
 THEN THE BASE PLATE EXTENDS 23-INCHES FROM THE
 RIGHT-OF-WAY LINE AND INTO THE SIDEWALK. IN ORDER TO
 HAVE THE 36-INCH CLEARANCE, THE WIDTH FROM THE INSIDE
 EDGE OF THE CURB TO THE RIGHT-OF-WAY LINE MUST BE SHOWN
 ON THE SURVEY AS AT LEAST 59-INCHES OR 4.92-FEET.
  
 4) FROM THE INFORMATION CURRENTLY PRESENTED, THE
 14-INCH DIAMETER POLE DOES NOT FIT AT THIS LOCATION.
  
 5) THE SUBMITTED PLANS SHOW THE UNDERGROUND ELECTRIC
 BEING SUPPLIED FROM AN FPL TRANSFORMER THAT IS LOCATED
 ON CITY PROPERTY ON THE EAST SIDE OF EVERNIA STREET.
 THE SURVEY NEEDS TO SHOW THAT THE AREA WHERE YOU PLAN
 TO PULL POWER FROM ACROSS CITY PROPERTY IS LOCATED
 WITHIN A UTILITY EASEMENT. IF NO EASEMENT IS PRESENT,

 THE POLE AND THE EDGE OF PAVEMENT:
  
 A. THE STEALTH POLE SHOWN ON SHEET C3, ELEVATION AND
 EQUIPMENT PLAN, HAS A 14-INCH DIAMETER. THE SURVEY
 SHOWS A FOUR (4) INCH DIAMETER POLE. SHEET C2, SITE
 PLAN, DOES NOT CALL OUT THE DIAMETER OF THE POLE, BUT
 SEEMS TO MIRROR THE SURVEY.
  
 B. THE SURVEY AND THE SITE PLAN SHOW A SIDEWALK WIDTH
 OF FOUR (4) FEET PLUS A SIX (6) INCH CURB FOR A TOTAL
 OF 54 INCHES IN WIDTH. A 14-INCH DIAMETER POLE OFFSET 7
 INCHES FROM THE EDGE OF THE RIGHT-OF-WAY RESULTS IN THE
 OUTSIDE EDGE OF THE POLE BEING 33 INCHES FROM THE
 TRAVEL LANE NOT 43 INCHES AS LISTED ON THE SURVEY. ADA
 REQUIRES A 36-INCH-WIDE FLUSH SURFACE, WHICH YOU DO NOT
 HAVE AT 33 INCHES. NOTE: THE FULL SIX (6) INCH CURB IS
 NOT CONSIDERED A FLUSH SURFACE SINCE THE OUTSIDE
 PORTION OF THE CURB SLOPES INTO A ROUNDED CONDITION
 WHERE IT TRANSITIONS TO THE ROADSIDE FACE OF THE CURB,

 REQUIRED.
  
 A. ALL UNDERGROUND UTILITIES NEED TO BE PROVIDED ON THE
 SURVEY. THE PROVIDED SURVEY SPECIFICALLY STATES THAT NO
 UNDERGROUND UTILITY LOCATES WERE DONE, WHICH DOES NOT
 COMPLY WITH THE CITY CODE REQUIREMENTS.
  
 B. THE ENGINEERING PLAN MUST BE BASED OFF OF THIS
 SURVEY INFORMATION.
  
 3) SURVEY OF ABOVEGROUND FACILITIES - A NEW POLE IS
 BEING INSTALLED ABOVEGROUND SO THIS IS REQUIRED.
  
 A. PLEASE NOTE THAT THE SURVEY DOES NOT PROVIDE THE
 SIZE OF THE EXISTING UTILITY POLE BASE DIAMETER, WHICH
 WILL NEED TO BE CORRECTED ON THE REVISED SURVEY.
  
 B. IDENTIFY ALL ABOVEGROUND COMMUNICATION FACILITIES IN
 THE AREA TO INCLUDE AN INVENTORY OF THE REGISTRANT'S
 EXISTING FACILITIES - NO INFORMATION ON EXISTING
 FACILITIES WAS PROVIDED. SHOW THIS INFORMATION ON THE
 PLANS AND PROVIDE A NARRATIVE. IF THERE ARE NO
 FACILITIES, THIS SHALL BE NOTED ON THE SURVEY.
  
 C. PLEASE NOTE: THE EXISTING DECORATIVE LIGHT SHOWN ON
 THE SURVEY IS 32 FEET TALL. THE SUPPORT POLE INCLUDED
 WITH THE APPLICATION IS 37 FEET IN HEIGHT, WHICH
 EXCEEDS THE ALLOWANCE LISTED IN SECTION 94-411(G)(1) OF
 THE CITY CODE. PROVIDE CLARIFICATION AS TO WHY THERE IS

 4) CONCEALMENT METHODS DESCRIPTIONS - NO CONCEALMENT
 INFORMATION WAS PROVIDED. PLEASE NOTE THAT THE PROPOSED
 SUPPORT POLE DOES NOT MEET THE CITY CONCEALMENT
 STANDARDS OUTLINED IN SECTION 78-411 OF THE CITY CODE.
 PROVIDE PLANS AND NARRATIVE ON HOW THE INSTALLATION
 COMPLIES WITH THE CITY'S CONCEALMENT REQUIREMENT.
  
 5) RESTORATION ESTIMATE - YOU MUST PROVIDE A
 RESTORATION ESTIMATE FOR THE SPECIFIC LOCATION OF THE
 POLE, NOT A GENERIC ESTIMATE THAT IS NOT LINKED TO THE
 SPECIFIC LOCATION AND WORK TO BE DONE.
  
 6) RADIO FREQUENCY INTERFERENCE - NO INFORMATION WAS
 PROVIDED THAT THE PROPOSED FACILITIES WILL NOT CAUSE
 RADIO FREQUENCY INTERFERENCE WITH THE CITY'S PUBLIC
 SAFETY COMMUNICATIONS EQUIPMENT. PROVIDE A STATEMENT
 REGARDING RADIO FREQUENCY INTERFERENCE.
  
 7) EVIDENCE OF INSURANCE COVERAGE
  
 A. THE CERTIFICATE OF LIABILITY INSURANCE LISTS ONLY
