2018-04-09 08:49:20APPLICATION WAS SUBMITTED WITHOUT ANY UTILITY RELEASES.
 TO HAVE THIS DEMO APPLICATION ISSUED, WE NEED THE
 FOLLOWING;
  
 1) COPY OF CONFIRMATION THAT FPL HAS REMOVED SERVICE TO
 THE BUILDING PROPOSED TO BE DEMOLISHED.
  
 2) COPY OF CONFIRMATION THAT FPU HAS REMOVED SERVICE TO
 THE BUILDING OR CONFIRM NONE WAS FOUND.
  
 3) COPY OF CONFIRMATION FROM THE WATER DEPARTMENT THAT
 A REQUEST WAS MADE TO PULL THE WATER METER. ON THE
 CONFIRMATION COPY, IT SHOULD BE NOTED HOW THE DEMO WILL
 BE KEPT WET. IF A HYDRANT METER IS RENTED, THAT WILL BE
 NOTED.
  
 4) A TURN-A-ROUND SEWER LATERAL CAPPING PERMIT WILL
 NEED TO BE PULLED. THIS WILL BE USED TO CAP THE SEWER
 LATERAL FROM THE BATHROOM. WHEN THE CAP IS COMPLETE, A
 FINAL PLUMBING #703 SHOULD BE REQUESTED AND PASSED.
  
 5) A COPY OF THE RODENT LETTER PROVIDED. CONTRACTOR IS
 TO HAVE A PEST CONTROL COMPANY VISIT SITE, INSPECT FOR
 RODENTS, AND TREAT FOR THEM IF FOUND.
  
 6) THE BUILDING IS COMMERCIAL. THEREFORE AN ASBESTOS
 ABATEMENT SURVEY WILL NEED TO BE COMPLETED. A COPY OF
 THE SUMMARY PROVIDED.
  
 7) A COPY OF THE N.O.C. SHOULD BE PROVIDED. THIS CAN BE
 DONE AT PERMIT PICKUP.
  
 8) A COPY OF THE DEBRIS DISPOSAL SHOULD BE PROVIDED.
 THIS ALSO CAN BE DONE AT PERMIT PICKUP.
  
 9) APPROVAL FROM THE HISTORIC AND ENGINEERING
 DEPARTMENTS. I WILL MOVE THE APPLICATION ON TO
 ENGINEERINGS MAILBOX NOW.
