| | ANNUAL INSPECTION ON 1/28/2016, THIS WAS AGAIN |
| | DISCUSSED AND CONFIRMED. A FINE OF 4 TIMES THE PERMIT |
| | FEE HAS BEEN ASSESSED FOR THIS PROJECT. |

| | 4) THIS PERMIT JOB VALUE IS UNDERVALUED. THE TOTAL |
