 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 1) THE AS BUILT CONDITIONS ARE NOT CONSISTENT WITH THE
 LATEST APPROVED REVISION TO THE ISSUED PERMIT. BASED ON
 THE ELEVATION CERTIFICATE, THERE ARE FOUR (4)
 NON-ENGINEERED FLOOD OPENINGS WITHIN 1.0 FOOT ABOVE
 ADJACENT GRADE IN THE ATTACHED GARAGE. PER THE
 ELEVATION CERTIFICATE THERE IS A TOTAL NET AREA OF
 FLOOD OPENINGS OF 548 SQIN. THE REVISED PLANS SHOW SIX
 (6) "SMART VENT" ENGINEERED FLOOD VENTS ON THREE
 DIFFERENT WALLS. SUBMIT REVISED DRAWINGS THAT SHOW THE
 AS BUILT CONDITIONS AND ARE CONSISTENT WITH THE
 ELEVATION CERTIFICATE.
  
 ONLY MATERIALS THAT ARE RESISTANT TO FLOOD DAMAGE ARE
 PERMITTED BELOW THE DFE (DESIGN FLOOD ELEVATION). THE
 DFE FOR THIS PROJECT IS 7.5' NGVD 1929.
  
 2) REGARDING THE ELEVATION CERTIFICATE PREPARED BY
 CLYDE O. MCNEAL, PLEASE ADDRESS THE FOLLOWING:
  
 A. THE OWNER'S NAME IS NOT CORRECT. THE OWNERS ARE
 JESUS J. AZQUETA JR. AND RACHEL AZQUETA.
  
 B. THE TWO LETTER STATE ABBREVIATION SHOULD BE ENTERED
 AT B3.
  
 C. THE SURVEYOR'S ORIGINAL HANDWRITTEN SIGNATURE IS
 REQUIRED ON THE CERTIFICATE.
  
 D. THE LATITUDE AND LONGITUDE THAT IS LISTED, PLACES
 THE POINT ON THE LOT TO THE NORTH OF THE PROJECT
 PROPERTY. THE LATITUDE FOR THIS LOT AT 105 GREGORY ROAD
 IS 26 DEGREES 38 MINUTES AND 45.86 SECONDS.
  
 E. PER THE FEMA 2012 EDITION OF THE ELEVATION
 CERTIFICATE INSTRUCTIONS, THE METHOD USED TO DETERMINE
 THE LATITUDE AND LONGITUDE IS REQUIRED TO BE INDICATED
 IN THE SECTION D COMMENT AREA.
  
 F. UNDER C2 THE ELEVATION DATUM USED FOR THE ELEVATIONS
 IN ITEMS A) THROUGH H) HAVE BOTH THE NGVD 1929 AND THE
 NAVD 1988 BOXES MARKED. THE INSTRUCTIONS REQUIRE THAT
 ALL ELEVATIONS MUST USE THE SAME DATUM ON WHICH THE BFE
 IS BASED, WHICH IS NGVD 1929 FOR THIS PROJECT. YOU ARE
 REQUIRED TO SHOW THE CONVERSION FROM THE FIELD SURVEY
 DATUM USED IF IT DIFFERS FROM THE DATUM USED FOR THE
 BFE AND INDICATE THE CONVERSION SOFTWARE USED IN THE
 COMMENTS AREA OF SECTION D.
  
 G) THE INSTRUCTIONS REQUIRE 3" X 3" COLOR PHOTOS OF ALL
 SIDES OF THE STRUCTURE THAT ARE IDENTIFIED AS "FRONT
 VIEW" , "REAR VIEW", "RIGHT SIDE VIEW" AND "LEFT SIDE
 VIEW". PHOTOGRAPHS OF THE FLOOD OPENINGS OR VENTS ARE
 REQUIRED.
  
 H) SUBMIT PHOTOGRAPHS OF ALL MECHANICAL AND UTILITY
 EQUIPMENT LOCATED OUTSIDE THE BUILDING SHOWING (1) ITS
 RELATION TO THE BUILDING AND GROUND AND (2) ITS
