 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 BUILDING
  
 1) THIS PRE-FIRM STRUCTURE IS LOCATED IN A SPECIAL
 FLOOD HAZARD AREA, SPECIFICALLY FLOOD ZONE A5 (EL 7 ).
 THE BASE FLOOD ELEVATION IS 7 FEET AND THE CITY OF WEST
 PALM BEACH HAS A 6 INCH FREEBOARD REQUIREMENT IN ITS
 FLOOD ORDINANCE. THE TOTAL VALUE OF WORK THAT HAS BEEN
 DONE TO THIS STRUCTURE INCLUDING THE PROPOSED WORK
 EXCEEDS 50 PERCENT OF THE STRUCTURE'S VALUE OF
 $134,770.00 LISTED ON THE PBC PROPERTY APPRAISER'S
 WEBSITE. THEREFORE, THE PROPOSED WORK IS A SUBSTANTIAL
 IMPROVEMENT AND THE LOWEST FINISHED FLOOR AND ANY
 EQUIPMENT SERVICING THE STRUCTURE IS REQUIRED TO BE AT
 OR ABOVE THE 7.5' ELEVATION. PROVIDE DOCUMENTATION TO
 SHOW THAT THE STRUCTURE IS IN COMPLIANCE OR HOW IT WILL
 BE BROUGHT INTO COMPLIANCE.
 THE CITY'S FLOOD ORDINANCE ARTICLE XVII. - FLOOD
 PREVENTION AND CONTROL SEC. 94-546. - PROVISIONS FOR
 FLOOD HAZARD REDUCTION.
 (A) AREAS OF SPECIAL FLOOD HAZARD GENERALLY. IN ALL
 AREAS OF SPECIAL FLOOD HAZARD, THE FOLLOWING PROVISIONS
 ARE REQUIRED:
 (1) ALL NEW CONSTRUCTION AND SUBSTANTIAL IMPROVEMENTS
 SHALL BE ANCHORED TO PREVENT FLOTATION, COLLAPSE OR
 LATERAL MOVEMENT OF THE STRUCTURE, AND SHALL BE
 CONSTRUCTED WITH MATERIALS AND UTILITY EQUIPMENT
 RESISTANT TO FLOOD DAMAGE BY METHODS AND PRACTICES THAT
 MINIMIZE FLOOD DAMAGE.
 (2) ALL NEW AND REPLACEMENT WATER SUPPLY SYSTEMS AND
 SANITARY SEWAGE SYSTEMS SHALL BE DESIGNED TO MINIMIZE
 OR ELIMINATE INFILTRATION OF FLOOD WATERS INTO THE
 SYSTEMS AND DISCHARGES FROM THE SYSTEMS INTO FLOOD
 WATERS; ON-SITE WASTE DISPOSAL SYSTEMS SHALL BE LOCATED
 TO AVOID IMPAIRMENT TO THEM OR CONTAMINATION FROM THEM
 DURING FLOODING.
 (3) ANY ALTERATION, REPAIR, RECONSTRUCTION OR
 IMPROVEMENTS TO A STRUCTURE ON WHICH THE START OF
 CONSTRUCTION WAS BEGUN AFTER MARCH 1, 1979, SHALL MEET
 THE REQUIREMENTS OF NEW CONSTRUCTION AS CONTAINED IN
 THIS ARTICLE.
 (B)AREAS OF SPECIAL FLOOD HAZARD WITH BASE FLOOD
 ELEVATION PROVIDED. IN ALL AREAS OF SPECIAL FLOOD
 HAZARD WHERE BASE FLOOD ELEVATION DATA HAS BEEN
 PROVIDED AS SET FORTH IN THIS ARTICLE, THE FOLLOWING
 PROVISIONS ARE REQUIRED:
 (1)RESIDENTIAL CONSTRUCTION. ALL NEW CONSTRUCTION OR
 SUBSTANTIAL IMPROVEMENT OF ANY EXISTING RESIDENTIAL
 STRUCTURE SHALL HAVE THE LOWEST FLOOR, INCLUDING
 BASEMENT, ELEVATED TO SIX INCHES ABOVE THE BASE FLOOD
 ELEVATION.
  
 SUBSTANTIAL IMPROVEMENT MEANS ANY COMBINATION OF
 REPAIRS, RECONSTRUCTION, ALTERATION, OR IMPROVEMENTS TO
 A STRUCTURE, TAKING PLACE DURING THE LIFE OF A
 STRUCTURE, IN WHICH THE CUMULATIVE COST EQUALS OR
 EXCEEDS 50 PERCENT OF THE MARKET VALUE OF THE
 STRUCTURE. THE MARKET VALUE OF THE STRUCTURE SHOULD BE
 THE APPRAISED VALUE OF THE STRUCTURE PRIOR TO THE START
 OF THE INITIAL REPAIR OR IMPROVEMENT, OR IN THE CASE OF
 DAMAGE, THE VALUE OF THE STRUCTURE PRIOR TO THE DAMAGE
 OCCURRING. FOR THE PURPOSES OF THIS DEFINITION,
 "SUBSTANTIAL IMPROVEMENT" IS CONSIDERED TO OCCUR WHEN
 THE FIRST ALTERATION OF ANY WALL, CEILING, FLOOR, OR
 OTHER STRUCTURAL PART OF THE BUILDING COMMENCES,
 WHETHER OR NOT THAT ALTERATION AFFECTS THE EXTERNAL
 DIMENSIONS OF THE STRUCTURE. THE TERM "SUBSTANTIAL
 IMPROVEMENT" DOES NOT, HOWEVER, INCLUDE ANY IMPROVEMENT
 OF A STRUCTURE TO COMPLY WITH EXISTING HEALTH,
 SANITARY, OR SAFETY CODES WHICH ARE SOLELY NECESSARY TO
 ENSURE SAFE LIVING CONDITIONS.
  
 2) THE PRODUCT APPROVAL FOR THE PGT MULLIONS, NOA NO.
 11-0922.01, HAS BEEN SUPERSEDED. SUBMIT TWO COPIES OF
 THE CURRENT NOA AS REQUIRED BY DCA RULE 9N-3.
  
 THE CUMLATIVE TOTAL OF WORK IS $74,218.00, WHICH IS
 MORE THAN 50 PERCENT OF THE IMPROVEMENT VALUE
 ($134,770.00) LISTED ON THE PBC PROPERTY APPRAISAL
 WEBSITE.
