 1- ACOUSTICAL UNDERLAYMENT: PROVIDE 2 COPIES OF THE
 FLAME SPREAD INDEX AND SMOKE-DEVELOPED INDEX IN
 ACCORDANCE WITH ASTM E-84 OR UL 723 AS REQUIRED BY SEC.
 719.1 OF 2010 FBC.
  
 2- ACOUSTICAL UNDERLAYMENT: PROVIDE 2 COPIES OF THE
 SOUND TRANSMISSION CLASS (STC) TEST IN ACCORDANCE WITH
 ASTM E 90 AND 2 COPIES OF THE IMPACT INSULATION CLASS
 (IIC) TEST IN ACCORDANCE WITH ASTM E 492 AS REQUIRED BY
 SEC. 1207 OF 2010 FBC.
  
 3- SPECIFY TYPE OF FINISH FLOORING TO BE INSTALLED.
 SEC. 107.2.1 CITY AMENDMENTS TO FBC.
  
 4- REVISE VALUATION: VALUATION DECLARED IN THE PERMIT
 APPLICATION IS TOO LOW. VALUATION TO INCLUDE ALL LABOR,
 MATERIALS AND CONTRACTOR'S OVERHEAD AND PROFIT AS
 REQUIRED BY SEC. 109.3 CITY AMENDMENTS TO FBC.
