 1. WHEN RESUBMITTING, PROVIDE TWO IDENTICAL COLLATED
 SETS OF PERMIT DOCUMENTS, FBC 107.
  
 AN ATTEMPT WAS MADE TO TRY TO PUT TOGETHER TWO SETS.
 SOME OF THE ENGINEERING PAGES PROVIDED ARE PHOTOCOPIES;
 A MIAMI DADE NOA (13-0419.05) IS LACKING PAGE NUMBERS
 AT THE BOTTOM. DUE TO HEAVY WORK LOAD, I AM UNABLE TO
 DO THIS FOR THE CONTRACTOR AT THIS TIME.
  
 2. ENGINEERING FOR THE ENHANCED FASTENING FOR THE ROOF
 RECOVER IS SPECIFIC TO THAT PRODUCT APPROVAL. SEE
 GENERAL LIMITATION #7, PROVIDE ENGINEERING FOR THE
 ENHANCED FASTENING REQUIRED FOR THE REROOF PRODUCT
 APPROVAL, FS471, FBC 1609.
  
 3. THE APPLICATION SUBMITTED VIA EMAIL FOR PRELIMINARY
 REVIEW LACKS THE ADDRESS AND THE OWNER'S SIGNATURE. THE
 CONTRACTOR CAN SIGN FOR OWNER IF THE CONTRACTOR HAS A
 NOTARIZED POWER OF ATTORNEY, FBC 107.
  
 4. WHEN RESUBMITTING, PLEASE NOTIFIY VIA EMAIL. INCLUDE
 "REROOF RESUBMITTED 14090452" IN THE SUBJECT LINE. I
 WILL PRIORITIZE THE RESUBMITTAL AND, IF POSSIBLE, DO

 1. AN UNSIGNED LETTER, NOT ON LETTERHEAD, FROM AN
 UNKNOWN INDIVIDUAL HAS STATED THAT THIS IS A RECOVER
 APPLICATION, AND AREAS FOR WHICH MOISTURE EXCEED
 ACCEPTABLE LEVELS WILL BE REROOFED. THIS IS IN CONFLICT
 WITH THE APPLICATION WHICH STATES "REROOF FLAT ROOF,
 7740 SF, METAL DECK, MRH 18'FT." A ROOF RECOVER NOA WAS
 SUBMITTED. PROVIDE AN APPLICATION, COMPLETED BY THE
 CONTRACTOR, WHICH INCLUDES THE CORRECT SCOPE OF WORK.
 IT APPEARS THAT ONE OF OUR STAFF MEMBERS COMPLETED THE
 APPLICATION AS THE CONTRACTOR SUBMITTED IT BLANK.
  
 2. PROVIDE A VERIFIABLE COPY OF THE CONTRACT FOR THIS
 JOB, FBC 109, FS489.
  
 3. PROVIDE A REROOF PRODUCT APPROVAL FOR THE AREAS
 WHICH WILL BE REROOFED RATHER THAN RECOVERED, WITH
 ENGINEERING FOR ENHANCED FASTENING IF APPLICABLE, FBC
 1609.
  
 4. DO ONE OF THE FOLLOWING:
  
 A. HAVE THE ENGINEER PROVIDE A PLAN SHOWING THE EXTENT
 OF AREA WHICH WILL REQUIRE A REROOF RATHER THAN A
 RECOVER. TWO OF THE HIGH MOISTURE AREAS ARE IN CLOSE
 PROXIMITY (APPEAR TO BE 10' AND 20' APART). WILL THE
 AREA BETWEEN THESE TWO AREAS BE TREATED AS A RECOVER OR
 WILL THEY BE REMOVED? THE PLAN SHOULD SHOW LOCATIONS
 WITH DIMENSIONS, AS WELL AS DIMENSIONS OF THE OVERALL
 ROOF. THE PLAN SHOULD BE CLEAR THAT THE REPAIR AREA IS
 LESS THAN 25%,
  
 OR
  
 B. OBTAIN AN "INSPECTION ONLY" PERMIT, $150 (USE THE
 COUNTY WIDE BUILDING PERMIT FORM, DESCRIPTON OF WORK IS
 "INSPECTION TO VERIFY ROOF AREA TO BE REPLACED IS LESS
 THAN 25% IN ORDER TO OBTAIN ROOF RECOVER PERMIT
 14090452." HALF OF THE PERMIT FEE ($75) WILL BE APPLIED
 TO THIS PERMIT APPLICATION. THE PURPOSE OF THIS
 INSPECTION WOULD BE TO VERIFY THAT AREA TO BE REPAIRED
 IS LESS THAN 25%, DISCUSS METHOD PROPOSED.

 1. MOISTURE SURVEY SHOWS UNACCEPTABLE MOISTURE LEVELS
 IN PORTIONS OF THE ROOF DECK. PERMIT APPLICATION STATES
 THIS IS A "REROOF" (REMOVAL OF EXISTING ROOF DOWN TO
 DECK, INSTALL NEW DECK) BUT THE PRODUCT APPROVAL
 SUBMITTED IS FOR A "RECOVER" (INSTALL A NEW ROOF OVER
 THE EXISTING ROOF).
  
 THE MOISTURE SURVEY SHOWS AREAS OF HIGH (UNACCEPTABLE)
 MOISTURE, BUT NO INFORMATION WAS PROVIDED AS TO HOW
 THIS WILL BE RESOLVED. SEE FBC 2010 EXISTING BUILDING
 611.3(1), REMOVAL OF EXISTING ROOF REQUIRED WHRE THE
 EXISTING ROOF OR ROOF COVERING IS WATER SOAKED (SEE
 CODE FOR FULL TEXT OF THIS SECTION),
  
 2. SEE FBC 2010 EXISTING BUILDING 611.1.1, NOT MORE
 THAN 25 PERCENT OF THE TOTAL ROOF AREA OR ROOF SECTION
 OF AN EXISTING BUILDING SHALL BE REPAIRED. ENGINEER IS
 TO DETERMINE THE PERCENTAGE OF EXISTING ROOF OR ROOF
 COVERING IS REQUIRED TO BE REPAIRED PRIOR TO THE
 PROPOSED RECOVER.
  
 3. THIS ADDRESS HAS AN EXPIRED PERMIT, 08080132. IF IT
 IS DETERMINED THAT A REROOF IS REQUIRED RATHER THAN A
 ROOF RECOVER, A MECHANICAL PERMIT WILL BE REQUIRED AS
 WELL AS A DETAIL AS TO HOW THE MECHANICAL EQUIPMENT
 WILL BE INSTALLED, FBC 107, FBC 1509.7.
  
 TWO OF THE FOUR MECHANICAL UNITS ARE LOCATED IN AREAS
 OF UNACCEPTABLE MOISTURE. IF IT IS DETERMINED THAT
 THESE AREAS CAN BE REPAIRED, REINSTALLATION OF THE
 MECHANICAL EQUIPMENT MAY BE REQUIRED. IF THIS IS THE
 CASE, A MECHANICAL SUB PERMIT WILL BE REQUIRED AS WELL
