 THIS PERMIT WAS ISSUED WITH PROVISOS. THE PROVISOS WERE
 SUPPOSED TO BE ADDRESSED PRIOR TO FIRST INSPECTION. I
 THINK I RELEASED THE INSPECTION HOLD SINCE SOMEONE CAME
 IN TO THE OFFICE TO SPEAK WITH ME AND WAS QUITE ANXIOUS
 TO GET AN INSPECTION.
  
 ONE WAS FOR A COPY OF THE CONTRACT. THIS MAY BE SENT
 VIA EMAIL. I DON'T HAVE A RECORD OF RECEIVING THIS.
  
 THE SECOND WAS FOR THE ELEVATION CERTIFICATE. THIS WAS
 REQUESTED DURING PLAN REVIEW AS IT IS EASIEST TO REVIEW
 AT THAT TIME. WHEN I REVIEW THEM LATER, I DO HAVE TO
 LOOK AT THE PLAN AGAIN TO CONFIRM INFORMATION.
  
 REGARDING THE FINISHED CONSTRUCTION ELEVATION
 CERTIFICATE, PLEASE HAVE THE SURVEYOR REVISE AS
 FOLLOWS:

 SERVICING THE BUILDING, PLANS SHOW TWO WATER HEATERS,
 PLEASE REVISE TO INCLUDE AND DESCRIBE IN COMMENTS

 HOW LATITUDE/LONGITUDE WAS DETERMINED (SEE
