 ON WEDNESDAY, SEPTEMBER 19, 2012.

 BACK. SHE WILL BE COPIED ON THE OFFICIAL E-MAIL
 NOTIFICATION OF THE ADDRESSING CHANGES ON SEPTEMBER 19,

 *LATER ON THAT DAY (9/19/2012), DIANE FULCHER INFORMED
 ME THAT THE BACK STRUCTURE WAS CONSIDERED AN "ACCESSORY
 STRUCTURE" (NOT A "LEGALLY PERMITTED STRUCTURE") TO THE
 FRONT MAIN BUILDING. I TRIED TO CALL TERESA, BUT WAS
 UNSUCCESSFUL IN MY ATTEMPT, SO E-MAILED HER. I REVOKED
 & RESCINDED MY PRIOR E-MAIL NOTIFICATION CHANGE TO
 EVERYONE (INSTRUCTED THEM TO REVERT THEIR DATABASES
 BACK & RELAYED DIANE'S MESSAGE TO THEM). THERE IS ONLY
 A SINGLE PRIMARY ADDRESS OF 410 34TH ST REFERENCED TO

 FORM. OWNER/AGENT TO SIGN AND DATE BOTTOM OF FORM ALSO.
 PERSON PREPARING THE CALCULATIONS TO COORDINATE
 INFORMATION WITH FLOOR PLANS SUBMITTED. THERE ARE NOT
 SHINGLES ON THIS HOUSE. MAKE SURE ENERGY CALCULATIONS
 COMPLY WITH THE REQUIREMENTS OF TABLE 402.1.1 OF 2010
 FBC-ENERGY CONSERVATION.
