 1. AS THIS IS A UNIT LOCATED IN A CONDOMINIUM, SOME OF
 THE DRYWALL TO BE REMOVED MAY BE A PART OF A FIRE RATED
 ASSEMBLY. SHOW TENANT SEPARATIONS ON THE PLAN,
 INCLUDING HORIZONTAL ASSEMBLIES IF APPLICABLE. REVISE
 PLAN TO SHOW ALL FIRE RESISTANCE RATED WALLS AND SHOW
 HOW THIS ISSUE WILL BE ADDRESSED SO THE ADJACENT
 UNIT(S) AFFECTED ARE NOT COMPROMISED.
  
 2. AS THIS IS A UNIT LOCATED IN A CONDOMINIUM, THERE
 ARE SOUND TRANSMISSION REQUIREMENTS FOR THE FLOOR AND
 TENANT SEPARATION WALLS FBC1207. PLEASE CLARIFY HOW
 THIS WILL BE MAINTAINED.
  
 3. THE INDIVIDUAL TAKING RESPONSIBILITY FOR THE CODE
 COMPLIANCE AND ACCURACY OF THE JOB IS TO PROVIDE
 HIS/HER PRINTED NAME, SIGNATURE, AND SEAL IF APPLICABLE

 4. A CONTRACTOR LICENSED TO PERFORM THIS SCOPE OF WORK
 IS REQUIRED TO OBTAIN PERMIT. SEE FS 468.8411,
 DEFINITIONS, "MOLD REMEDIATION" MEANS MEANS THE
 REMOVAL, CLEANING, SANITIZING, DEMOLITION, OR OTHER
 TREATMENT, INCLUDING PREVENTIVE ACTIVITIES, OF MOLD OR
 MOLD-CONTAMINATED MATTER OF GREATER THAN 10 SQUARE FEET
 THAT WAS NOT PURPOSELY GROWN AT THAT LOCATION; HOWEVER,
 SUCH REMOVAL, CLEANING, SANITIZING, DEMOLITION, OR
 OTHER TREATMENT, INCLUDING PREVENTIVE ACTIVITIES, MAY
 NOT BE WORK THAT REQUIRES A LICENSE UNDER CHAPTER 489
 UNLESS PERFORMED BY A PERSON WHO IS LICENSED UNDER THAT
 CHAPTER OR THE WORK COMPLIES WITH THAT CHAPTER. DRYWALL
 WORK AND OTHER ITEMS INCLUDED ON THE PROPOSAL,
 INCLUDING DEMOLITION, REQUIRES A CONTRACTOR LICENSE,
 SEE FS489. FOR YOUR CONVENIENCE, THE APPLICABLE PORTION
 IS AS FOLLOWS:
  
 FLORIDA STATUTE 489, DEFINITIONS: "CONTRACTOR" MEANS
 THE PERSON WHO IS QUALIFIED FOR, AND SHALL ONLY BE
 RESPONSIBLE FOR, THE PROJECT CONTRACTED FOR AND MEANS,
 EXCEPT AS EXEMPTED IN THIS PART, THE PERSON WHO, FOR
 COMPENSATION, UNDERTAKES TO, SUBMITS A BID TO, OR DOES
 HIMSELF OR HERSELF OR BY OTHERS CONSTRUCT, REPAIR,
 ALTER, REMODEL, ADD TO, DEMOLISH, SUBTRACT FROM, OR
 IMPROVE ANY BUILDING OR STRUCTURE.
  
 IF ANY INFORMATION TO THE CONTRARY IS AVAILABLE (SUCH
 AS A DBPR LETTER), PLEASE PROVIDE.
  
 5. THE PROPOSAL PROVIDED TO THE PROPERTY OWNER INCLUDES
 ADDITIONAL WORK WHICH REQUIRES A BUILDING PERMIT.
 REVISE APPLICATION AND PLAN TO SHOW COMPLETE SCOPE OF
 WORK PROPOSED. FOR CLARIFICATION, PROVIDE A COPY OF THE
 MOLD ASSESSOR'S REPORT.
  
 6. MOLD REMEDIATION COMPANIES REQUIRE CERTIFICATION
 THROUGH DBPR, FLORIDA ADMINISTRATIVE CODE 61-31. PLEASE
 PROVIDE THE LICENSE NUMBER FOR ETS ENVIRONMENT INC. AS
 I WAS UNABLE TO LOCATE THIS INFORMATION ON THE DBPR
 WEBSITE.
