 ARE REQUIRED TO BE SIGNED AND DATED BY THE PREPARER AND
 THE OWNER/AGENT.
  
 3) SPECIFY THE UNDER FLOOR INSULATION THAT WILL BE
 INSTALLED ON THE PLANS.
  
 4) SHEET AO SPECIFIES SPRAY FOAM INSULATION IN THE
 ROOF. SUBMIT TWO COPIES OF THE MANUFACTURER'S
 SPECIFICATIONS / EVALUATION REPORTS.
  
 5) SPECIFY THE TERMITE TREATMENT THAT WILL BE USED ON

 6) FBC R319 REQUIRES WOOD FLOOR JOISTS THAT ARE CLOSER
 THEN 18 INCHES TO EXPOSED GROUND IN CRAWL SPACES TO BE

 7) IMPACT FEES MUST BE PAID TO PALM
 BEACH COUNTY, PLANS STAMPED BY THEM AND
 COPY OF RECEIPT SUBMITTED TO CITY OF
