 9) SHOW COMPLIANCE WITH R320.1 TERMITE PROTECTION SHALL
 BE PROVIDED BY REGISTERED TERMITICIDES, INCLUDING SOIL
 APPLIED PESTICIDES, BAITING SYSTEMS, AND PESTICIDES
 APPLIED TO WOOD, OR OTHER APPROVED METHODS OF TERMITE
 PROTECTION LABELED FOR USE AS A PREVENTATIVE TREATMENT
 TO NEW CONSTRUCTION (SEE SECTION 202, REGISTERED
 TERMITICIDE). UPON COMPLETION OF THE APPLICATION OF THE
 TERMITE PROTECTIVE TREATMENT, A CERTIFICATE OF
 COMPLIANCE SHALL BE ISSUED TO THE BUILDING DEPARTMENT
 BY THE LICENSED PEST CONTROL COMPANY THAT CONTAINS THE
 FOLLOWING STATEMENT: THE BUILDING HAS RECEIVED A
 COMPLETE TREATMENT FOR THE PREVENTION OF SUBTERRANEAN
 TERMITES. TREATMENT IS IN ACCORDANCE WITH RULES AND
 LAWS ESTABLISHED BY THE FLORIDA DEPARTMENT OF
 AGRICULTURE AND CONSUMER SERVICES.
  
 10) SHOW COMPLIANCE WITH R806.1 VENTILATION REQUIRED.
 ENCLOSED ATTICS AND ENCLOSED RAFTER SPACES FORMED WHERE
 CEILINGS ARE APPLIED DIRECTLY TO THE UNDERSIDE OF ROOF
 RAFTERS SHALL HAVE CROSS VENTILATION FOR EACH SEPARATE
 SPACE BY VENTILATING OPENINGS PROTECTED AGAINST THE
 ENTRANCE OF RAIN. VENTILATING OPENINGS SHALL BE
 PROVIDED WITH CORROSION-RESISTANT WIRE MESH, WITH 1/8
 INCH (3.2 MM) MINIMUM TO 1/4 INCH (6.4 MM) MAXIMUM
 OPENINGS. A CONTINUOUS EAVE VENT WILL BE REQUIRED.
  
 11) SPECIFY THE LENGTH/EMBED DEPTH OF THE 3/4" ANCHOR
 BOLTS FOR THE BOTTOM PLATE. R403.1.6 REQUIRES A MINIMUM
 OF 7" INTO CONCRETE.
  
 12) SUBMIT TWO COPIES OF THE SURVEY. FBC 106.1* CITY
 AMENDMENTS
  
 13) SUBMIT TWO COPIES OF ENERGY CALCULATION FORMS AS
 REQUIRED BY FBC 13-6.
  
 14) SUBMIT TWO COPIES OF PRODUCT APPROVALS. DCA RULE

 15) FBC 106.3* CITY AMENDMENTS PRODUCT APPROVALS. THOSE
 PRODUCT WHICH ARE REGULATED BY DCA RULE 9B-72 SHALL BE
 REVIEWED AND APPROVED IN WRITING BY THE DESIGNER OF
 RECORD PRIOR TO SUBMITTAL FOR JURISDICTIONAL APPROVAL.
  
 16) SUBMIT PRODUCT APPROVALS FOR THE BUILT-UP ROOF
 SYSTEM SPECIFIED ON THE PLANS. THE ROOFING PRODUCT
 APPROVAL THAT WAS SUBMITTED IS FOR ASPHALT SHINGLES
 WHICH CAN NOT BE USED ON A FLAT ROOF.
  
 17) THE PRODUCT APPROVAL THAT WAS SUBMITTED FOR THE
 OUTSWING DOORS IS FOR AN IMPACT RATED GLAZED DOOR.
 SUBMIT THE PRODUCT APPROVALS FOR THE STORAGE ROOM DOOR,
 UNLESS THIS IS THE TYPE OF DOOR THAT WILL BE INSTALLED
 ON THE STORAGE ROOM. DCA RULE 9B-72
  
 NOTE: THE PRODUCT APPROVALS THAT WERE SUBMITTED FOR THE
 WINDOWS AND GLASS DOOR ARE LARGE MISSILE IMPACT RATED
 AND DO NOT REQUIRE STORM PANELS. HOWEVER, IF IMPACT
 WINDOWS AND DOORS WILL NOT BE USED, PRODUCT APPROVALS
