 AND EFFICIENCIES INSTALLED MUST MEET OR EXCEED THE
 MINIMUM VALUES LISTED ON THE FORM. COMPONENTS AND
 EQUIPMENT NOT BEING INSTALLED OR RENOVATED MAY BE LEFT
 BLANK. THE R-VALUE OF 4.2 SPECIFIED ON SHEETS 3 AND 4
 OF THE PLANS DOES NOT MEET THE MINIMUM OF R-5 REQUIRED
 BY THE ENERGY FORM. IF THE CENTRAL A/C IS NOT BEING
 ADDED OR RENOVATED AS INDICATED IN THE LETTER THAT WAS
 SUBMITTED LEAVE THOSE LINES BLANK. THE OWNER/AGENT IS
 REQUIRED TO SIGN THE FORM. SEE FBC CHAPTER 13.
  
 2) THE NOTES ON SHEET 1 SPECIFY A NEW 4" SLAB POURED
 OVER THE EXISTING SLAB. ON SHEET 2 A NEW 6" SLAB IS
 SPECIFIED OVER THE EXISTING SLAB. ON SHEET 1 THE
 EXISTING SLAB IS SHOWN AT 4" BELOW THE FINISH FLOOR
 ELEVATION OF THE LIVING AREA. IT APPEARS THAT THE
 EXISTING SLAB WILL NEED TO BE REMOVED IN ORDER TO
 INSTALL THE 6" STRUCTURAL SLAB REQUIRED DUE TO THE POOR
 SOIL CONDITIONS. PLEASE REVIEW AND MAKE
 CORRECTIONS.CITY AMENDMENTS TO FBC 106.1.2.
  
 3) SUBMIT PRODUCT APPROVALS FOR THE MULLIONS FOR THE
 TWO WINDOWS THAT ARE MULLED TOGETHER. FAC RULE 9B-72.
  
  
 4) THE PRODUCT APPROVAL THAT WAS SUBMITTED FOR THE
 EXTERIOR DOOR IS EXPIRED AND IS FOR A NON-IMPACT GLAZED
 OUTSWING DOOR. THE PLANS INDICATE A SIX PANEL METAL
 DOOR WITH NO GLASS AND THE STORM PANEL SCHEDULE DOES
 NOT LIST THIS DOOR. CHANGE THE PRODUCT APPROVAL OR THE
 PLANS FOR THIS DOOR. IF THIS IS A GLAZED DOOR, SPECIFY
 THE STORM PANELS OR OTHER IMPACT PROTECTION REQUIRED BY
 R301.2.1.2.
  
 5) ON THE PRODUCT APPROVAL FOR THE ROOF COVERING
 SPECIFY THE APPROVED ASSEMBLY AND FASTENING ASSEMBLY
 THAT WILL BE INSTALLED. PROVIDE RAS 117 CALCULATIONS
 FOR ENHANCED FASTENING IF PRESSURES ARE TOO LOW. CITY
 AMENDMENTS TO FBC 106.1.2.
  
 6) THE STORM PANEL SCHEDULE THAT WAS SUBMITTED DOES NOT
 CORRESPOND WITH THE PRODUCT APPROVAL THAT WAS
 SUBMITTED. THE 40" SPAN FOR OPENING #2 IS LESS THAN THE
 OPENING SIZE. THE ANCHOR IN THE TABLE SHOULD BE
 PANELMATE PLUS NOT TAPCON. CITY AMENDMENTS TO FBC
 106.1.2.
  
 NOTE: IN ORDER TO REUSE EXISTING DOORS AND WINDOWS,
 THEY MUST HAVE CURRENT PRODUCT APPROVALS. IF THE
 PRODUCT APPROVALS THAT ARE SUBMITTED AND REVIEWED DO
 NOT MATCH THE PRODUCTS THAT ARE BEING REINSTALLED THEY
 CAN NOT BE USED. THEY WILL BE VERIFIED BY THE INSPECTOR
 IN THE FIELD AND WILL HAVE TO BE REPLACED IF THEY DON'T
