 CALCULATION FORM SIGNED BY THE OWNER/AGENT.

 COPY OF RECEIPT SUBMITTED TO CITY OF

 3) SHOW COMPLIANCE WITH SECTION 704 INSPECTION FOR
 TERMITES.
 IN ORDER TO PROVIDE FOR INSPECTION FOR TERMITE
 INFESTATION, CLEARANCE BETWEEN EXTERIOR WALL COVERINGS
 AND FINAL EARTH GRADE ON THE EXTERIOR OF A BUILDING
 SHALL NOT BE LESS THAN 6 INCHES (152 MM). EXCEPTIONS:
 1.PAINT OR DECORATIVE CEMENTITIOUS FINISH LESS
 THAN 5 / 8 INCH (17.1 MM) THICK ADHERED DIRECTLY TO THE
 MASONRY FOUNDATION SIDEWALL.
 2.ACCESS OR VEHICLE RAMPS WHICH RISE TO THE
 INTERIOR FINISH FLOOR ELEVATION FOR THE WIDTH OF SUCH
 RAMPS ONLY.
 3.A 4-INCH (102 MM) INSPECTION SPACE ABOVE PATIO
 AND GARAGE SLABS AND ENTRY AREAS.
 4.IF THE PATIO HAS BEEN SOIL TREATED FOR TERMITES,
 THE FINISH ELEVATION MAY MATCH THE BUILDING INTERIOR
 FINISH FLOOR ELEVATIONS ON MASONRY CONSTRUCTION ONLY.
 5.MASONRY VENEERS.
  
 4) SHOW COMPLIANCE WITH R404.1.6 HEIGHT ABOVE FINISHED
 GRADE.
 CONCRETE AND MASONRY FOUNDATION WALLS SHALL EXTEND
 ABOVE THE FINISHED GRADE ADJACENT TO THE FOUNDATION AT
 ALL POINTS A MINIMUM OF 4 INCHES (102 MM) WHERE MASONRY
 VENEER IS USED AND A MINIMUM OF 6 INCHES (152 MM)
 ELSEWHERE.
  
 5) SHOW COMPLIANCE WITH R319.1.
 IN AREAS SUBJECT TO DECAY DAMAGE AS ESTABLISHED BY
 TABLE R301.2(1) , THE FOLLOWING LOCATIONS SHALL REQUIRE
 THE USE OF AN APPROVED SPECIES AND GRADE OF LUMBER,
 PRESSURE TREATED IN ACCORDANCE WITH AWPA C1, C2, C3,
 C4, C9, C15, C18, C22, C23, C24, C28, C31, C33, P1, P2
 AND P3, OR DECAY-RESISTANT HEARTWOOD OF REDWOOD, BLACK
 LOCUST, OR CEDARS.
 2.ALL WOOD FRAMING MEMBERS THAT REST ON CONCRETE
 OR MASONRY EXTERIOR FOUNDATION WALLS AND ARE LESS THAN
 8 INCHES (203 MM) FROM THE EXPOSED GROUND.
 5.WOOD SIDING, SHEATHING AND WALL FRAMING ON THE
 EXTERIOR OF A BUILDING HAVING A CLEARANCE OF LESS THAN
 6 INCHES (152 MM) FROM THE GROUND.
  
 6) PROVIDE EXTERIOR ELEVATIONS. FBC* 106.1.2.
  
 7) PROVIDE DETAIL OF FRAMING AND STRAPPING AT EXTERIOR
 WINDOWS.
  
 8) THE BOTTOM PLATE IS NOT SHOWN ON THE WALL SECTION ON
 SHEET 2.
  
 9)PRODUCT APPROVALS SUBMITTED WITH
 PERMIT APPLICATION AFTER OCTOBER 1, 2003
 ARE REQUIRED TO COMPLY WITH THE FLORIDA

 MULLIONS USED TO CONNECT THE SINGLE HUNG WINDOWS TO THE
 FIXED WINDOWS ABOVE THEM.
  
 12) THE PRODUCT APPROVALS FOR THE PGT FRENCH DOORS ARE
 MISSING SOME OF THE PAGES OF DRAWING AND SOME ARE
 MISSING A PORTION OF THE TEXT.
