 MUST INCLUDE THE NAME & ADDRESS &
 LICENSE NUMBER. SEE COPY OF STATE
 STATUTE ATTACHED.
 3. A NOTICE OF COMMENCEMEMT MUST BE
 FILED AT THE COUNTY CLERKS OFFICE AND
 RECORDED. IT MUST BE SUBMITTED BEFORE A
 PERMIT CAN BE ISSUED.
 4. SEPARATE PERMITS & PLANS WILL BE
 REQUIRED FOR ANY PLUMBING AND ELECTRICAL
