 PERMIT 20060481 AT&T CRAN_RTFL_PBC36_01 4 916 5TH ST
  
 THE PROPOSED POLE LOCATION IS IN A RESIDENTIAL AREA. IN
 ACCORDANCE WITH SECTION 78-411(B)(2) OF THE CITY CODE,
 WHENEVER WIRELESS FACILITIES MUST BE PLACED IN A
 RIGHT-OF-WAY WITH RESIDENTIAL USES ON ONE OR BOTH
 SIDES, NEITHER POLES, EQUIPMENT, ANTENNAS OR OTHER
 STRUCTURES SHALL BE PLACED DIRECTLY IN FRONT OF A
 RESIDENTIAL STRUCTURE. ALL WIRELESS FACILITIES SHALL BE
 LOCATED SUCH THAT VIEWS FROM RESIDENTIAL STRUCTURES ARE
 NOT SIGNIFICANTLY IMPAIRED. THANK YOU FOR MOVING THE
 POLE FROM IN FRONT OF THE RESIDENTIAL STRUCTURE AT 908
 5TH STREET; HOWEVER, THE NEW PROPOSED LOCATION IS NOW
 PROPOSED SUCH THAT IT WILL BE IN FRONT OF A FUTURE
 RESIDENTIAL STRUCTURE AT 916 5TH STREET. SINCE THIS
 SEEMS TO BE YOUR DESIRED LOCATION FOR THE POLE, IN
 ORDER TO COMPLY WITH CITY REQUIREMENTS, THE POLE NEEDS
 TO BE INSTALLED IN THE PARKWAY AREA AT THE PROPERTY
 LINE EXTENDED BETWEEN 908 AND 916 5TH STREET. THE
 PROPERTY LINE NEEDS TO BE CLEARLY SHOWN ON THE SURVEY
 SO THAT STAFF CAN VERIFY LOCATION COMPLIANCE.
  
 THE SURVEY SHOWS THAT THE POLE WILL BE WITHIN FIVE (5)
 FEET OF THE EXISTING WATER CONNECTIONS AND METERS FOR
 THE 916 5TH STREET PROPERTY. THE POLE MAY NOT BE THIS
 CLOSE TO THE WATER LINES AND REINFORCES THE MOVE
 EASTWARD TO THE PROPERTY LINE EXTENDED.
  
 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) ENGINEERING PLAN - NO UPDATED ENGINEERING PLAN WAS
 SUBMITTED.
  
 A. THE ENGINEERING PLANS DID NOT INCLUDE A SCALE. THE
 CITY CODE REQUIRES THE PLANS TO BE DRAWN AT A 10FT,
 20FT, OR 40FT SCALE. PROVIDE THE PLANS AT THE REQUIRED
 SCALE.
  
 B. THE SUBMITTED ENGINEERING PLAN MUST BE BASED ON THE
 SURVEY AND SHOW THE ABOVE AND BELOW GROUND FEATURES.
 THIS INCLUDES THE TREES ALONG THE RIGHT-OF-WAY.
  
 C. THE SIZE OF THE PROPOSED POLE BASE WITHIN THE
 RIGHT-OF-WAY MUST BE SHOWN ON THE ENGINEERING PLAN.
  
 D. YOU ARE REQUIRED TO PROVIDE INFORMATION REGARDING
 EXISTING UTILITY POLES (INCLUDING LIGHT POLES) WITHIN
 50 FEET OF YOUR PROPOSED POLE. THE POLE HEIGHT AND BASE
 DIAMETER SHALL BE INCLUDED. THIS WILL BE USED TO VERIFY
 IF THE PROPOSED 25 FOOT SUPPORT POLE (WITH 5 FOOT
 ANTENNA CANNISTER) HEIGHT COMPLIES WITH THE HEIGHTS OF
 EXISTING UTILITY POLES.
  
 E. THE ENGINEERING PLAN MUST BE BASED OFF OF THIS
 SURVEY INFORMATION.
  
 2) THANK YOU FOR USING A POLE STYLE THAT MEETS THE
 CITY'S CONCEALMENT REQUIREMENTS. AT THIS LOCATION, THE
 CITY REQUIRES THE POLE TO BE EITHER BLACK OR GREEN, NOT
 ITS NATURAL STEEL COLOR. PLEASE SPECIFY THE COLOR YOU
 CHOOSE TO USE ON THE PLANS.
  
 3) EVIDENCE OF INSURANCE COVERAGE - YOU ARE REQUIRED TO
 SUBMIT VERIFICATION OF INSURANCE IN ACCORDANCE WITH THE
 PROVISIONS OF SECTION 78-406 OF THE CITY CODE OF
 ORDINANCES. PLEASE REMEMBER THAT THE CERTIFICATE OF
 LIABILITY INSURANCE NEEDS TO BE ISSUED TO THE APPLICANT
 LISTED ON THE COMMUNICATIONS FACILITIES PLACEMENT
 PERMIT APPLICATION - NEW CINGULAR WIRELESS LLC.
  
 4) ELECTRONIC FORMAT OF SUBMITTAL - THE SUBMITTAL WAS
 NOT PROVIDED IN ELECTRONIC FORM. A COPY OF ALL OF THE
 SUBMITTAL MATERIAL MUST BE SUBMITTED IN ELECTRONIC
 FORMAT.
