 IN ITS PLACE. PLEASE SUBMIT SHEET 17B.
  
 2) REPEAT COMMENT: PROVIDE A SURVEY, PREPARED BY
 FLORIDA LICENSED SURVEYOR, FOR SHEETS 22 AND 23.
  
 A. THE BORE ON SHEET 22 WAS SHIFTED TO SHOW FURTHER
 AWAY FROM THE CORNER OF THE PRIVATE PROPERTY THAT IS
 SHOWN ON THE ENGINEERING PLAN ENCROACHING INTO THE
 SIDEWALK. THE SURVEY OF THIS LOCATION IS REQUIRED SO
 THAT STAFF CAN VERIFY THAT THE BORE AT THIS LOCATION IS
 NOT ON PRIVATE PROPERTY OR THAT THERE IS A UTILITY
 EASEMENT IF THE BORE ENCROACHES.
  
 B. THE BORE ON SHEET 23 STILL SHOWS ENCROACHMENT ON
 PRIVATE PROPERTY (OWNED BY FPL). UNLESS THERE IS A
 UTILITY EASEMENT SHOWN ON THE SURVEY AT THESE
 LOCATIONS, YOU MAY NOT CROSS ONTO PRIVATE PROPERTY.
 NOTE: BASED ON REVIEWING THE PALM BEACH COUNTY PROPERTY
 APPRAISER SITE, THE SIDEWALK IS NOT WHOLLY WITHIN THE
 RIGHT-OF-WAY; THEREFORE, YOUR BORE UNDER THE SIDEWALK
 CROSSES ON PRIVATE PROPERTY, WHICH IS NOT PERMITTED.
 THE SURVEY IS REQUIRED TO VERIFY THAT EITHER YOU ARE
 NOT ON PRIVATE PROPERTY OR THAT THERE IS A UTILITY
 EASEMENT FOR WHERE YOU ENCROACH.
  
 3) THE NEW PLANS SHOW THE BORE ROUTE PROCEEDING ALONG
 SAPODILLA AVENUE AND FERN STREET (SHEETS 25, 27, 28,
 29, 30 INSTEAD OF ROSEMARY AVENUE AS IN THE PREVIOUS
 SUBMITTALS. PROVIDE A SURVEY, PREPARED BY FLORIDA
 LICENSED SURVEYOR, FOR THE NEW ROUTE ON SAPODILLA
 AVENUE.
  
 4) THE NEW BORE ON THE NORTH SIDE OF OKEECHOBEE
 BOULEVARD (SHEET 26), JUST WEST OF ROSEMARY AVENUE, IS
 VERY CLOSE TO THE PROPERTY LINE. PROVIDE A SURVEY,
 PREPARED BY FLORIDA LICENSED SURVEYOR, OF THIS LOCATION
 IS REQUIRED SO THAT STAFF CAN VERIFY THAT THE BORE AT
 THIS LOCATION IS NOT ON PRIVATE PROPERTY OR THAT THERE
 IS A UTILITY EASEMENT IF THE BORE ENCROACHES.
  
 5) NO SHEET 29 IS PROVIDED, YET THERE ARE TWO SHEET
 30S. THE LOCATION MAP SHOWS THE INTERSECTION OF
 SAPODILLA AVENUE AND FERN STREET AS SHEET 29, YET IS IT
 LISTED AS SHEET 30. LOCATION MAP SHEET 30 FOR THE
 INTERSECTION OF FERN STREET AND ROSEMARY AVENUE IS NOT
 PROVIDED. IT APPEARS THE CROSS SECTIONS MAY BE PROPERLY
 LABELED.
  
 6) UPDATED COMMENT: ENGINEERING SHEET 30 WILL NEED TO
 SHOW THAT ANY BORE PITS AND HANDHOLE ON THE NORTH AND
 SOUTH SIDES OF FERN STREET AS WELL AS THE BORES, DO NOT
 DISTURB THE STRUCTURAL SOILS THAT EXTEND FIVE (5) FEET
 OUT FROM THE EDGE OF THE TREE PITS. ADDITIONALLY, NO
 UPDATED SURVEY SHOWING THE SOUTH SIDE OF FERN STREET IN
 THE LOCATION OF WORK WAS PROVIDED. CURRENTLY THE
 SURVEYOR'S NOTES COVER THE AREA ON THE LAST SURVEY THAT
 STAFF HAS ON FILE.
  
 7) THE BORE SHOWN AT 5 TO 12 FEET BELOW GRADE IS AT A
 SUFFICIENT DEPTH TO AVOID TREE AND PALM DAMAGE.
  
 8) THE INSTALLATION CONTRACTOR MUST CONTACT SUNSHINE
 811 TO VERIFY THE LOCATION OF ALL UTILITIES AND VERIFY
 ALL DEPTHS PRIOR TO CONSTRUCTION. ALL CITY UTILITIES
 MUST BE LOCATED BY SOFT DIG METHODS BEFORE STARTING THE
 WORK.
  
 9) THE RIGHT-OF-WAY USE PERMIT REQUIRES YOU TO PROVIDE
 A CERTIFICATE OF LIABILITY INSURANCE, WHICH IN
 ACCORDANCE WITH SECTION 78-406 OF THE CITY CODE
 SPECIFICALLY NAMES THE CITY OF WEST PALM BEACH, ITS
 COMMISSIONERS, OFFICERS, EMPLOYEES AND AGENTS AS
 ADDITIONAL INSURED.
