 1) REPEAT COMMENT: PROVIDE SURVEY FOR SHEETS 22 AND 23.
  
 A. THE BORE ON SHEET 22 WAS SHIFTED TO SHOW FURTHER
 AWAY FROM THE CORNER OF THE PRIVATE PROPERTY THE IS
 SHOWN ON THE ENGINEERING PLAN ENCROACHING INTO THE
 SIDEWALK. THE SURVEY OF THIS LOCATION IS REQUIRED SO
 THAT STAFF CAN VERIFY THAT THE BORE AT THIS LOCATION IS
 NOT ON PRIVATE PROPERTY OR THAT THERE IS A UTILITY
 EASEMENT IF THE BORE ENCROACHES.
  
 B. THE BORE ON SHEET 23 STILL SHOWS ENCROACHMENT ON
 PRIVATE PROPERTY (OWNED BY FPL). UNLESS THERE IS A
 UTILITY EASEMENT SHOWN ON THE SURVEY AT THESE
 LOCATIONS, YOU MAY NOT CROSS ONTO PRIVATE PROPERTY.
 NOTE: BASED ON REVIEWING THE PALM BEACH COUNTY PROPERTY
 APPRAISER SITE, THE SIDEWALK IS NOT WHOLLY WITHIN THE
 RIGHT-OF-WAY; THEREFORE, YOUR BORE UNDER THE SIDEWALK
 CROSSES ON PRIVATE PROPERTY, WHICH IS NOT PERMITTED.
 THE SURVEY IS REQUIRED TO VERIFY THAT EITHER YOU ARE
 NOT ON PRIVATE PROPERTY OR THAT THERE IS A UTILITY
 EASEMENT FOR WHERE YOU ENCROACH.
  
 2) REPEAT COMMENT: ENGINEERING SHEET 29 SHOWS A NEW
 HANDHOLE IN THE SIDEWALK ON THE NORTH AND SOUTH SIDES
 OF FERN STREET.
  
 A. HANDHOLE 99+60 ? THANK YOU FOR PROVIDING THE UPDATED
 RIGHT-OF-WAY CONFIGURATION ON THE ENGINEERING PLAN
 INCLUDING DIMENSIONS. THE NEW TREE PITS ARE SURROUNDED
 BY A LARGER AREA OF STRUCTURAL SOIL, WHICH YOU ARE NOT
 PERMITTED TO DISTURB WITH THE DIGGING AND INSTALLATION
 OF THE HANDHOLE. THE STRUCTURAL SOIL EXTENDS FIVE (5)
 FEET OUT FROM THE EDGE OF THE TREE PIT. NOTE: SHEET 29
 CALLS THIS OUT AS 99+60; HOWEVER, SHEETS 29B AND 29C
 REFERENCE THIS HANDHOLE AS 1+61. ALL SHEETS MUST SHOW
 THE SAME, ACCURATE INFORMATION. THE FINAL RIGHT-OF-WAY
 CONFIGURATION NEEDS TO BE SHOWN ON THE SURVEY SO STAFF
 CAN VERIFY WHAT IS SHOWN ON THE ENGINEERING PLANS.
  
 B. NO UPDATED SURVEY SHOWING THE SOUTH SIDE OF FERN
 STREET IN THE LOCATION OF WORK WAS PROVIDED. CURRENTLY
 THE SURVEYOR'S NOTES COVER THE AREA.
  
 3) PLEASE NOTE THAT CITYPLACE RETAIL, LLC, IS IN THE
 PROCESS OF RECONFIGURING THE SIDEWALK/LANDSCAPE WITHIN
 THE OKEECHOBEE BOULEVARD RIGHT-OF-WAY AT THE LOCATION
 OF HANDHOLE 1+69. YOU WILL NEED TO VERIFY THE FINAL
 CONDITIONS TO COORDINATE YOUR INSTALLATION. STAFF IS
 CONCERNED WITH THE FINAL GRADE OF THE HANDHOLE IN
 RELATIONSHIP TO THE SURROUNDING AREA. **** HAVE YOU
 COORDINATED WITH CITYPLACE RETAIL?
  
 4) PLEASE NOTE THAT THE PROPOSED TIE IN AT 82+59 HAS
 SHRUBS AND OTHER PLANTINGS THAT MUST BE REPLACED (NOT
 GRASS AS STATED). THE EDGE OF THE TIE IN LOCATION MUST
 BE AT LEAST EIGHT (8) FEET FROM THE TRUNK OF THE TREE.
  
 5) THE BORE SHOWN AT 5 TO 12 FEET BELOW GRADE IS AT A
 SUFFICIENT DEPTH TO AVOID TREE AND PALM DAMAGE.
  
 6) THE INSTALLATION CONTRACTOR MUST CONTACT SUNSHINE
 811 TO VERIFY THE LOCATION OF ALL UTILITIES AND VERIFY
 ALL DEPTHS PRIOR TO CONSTRUCTION. ALL CITY UTILITIES
 MUST BE LOCATED BY SOFT DIG METHODS BEFORE STARTING THE
 WORK.
  
 7) THE RIGHT-OF-WAY USE PERMIT REQUIRES YOU TO PROVIDE
 A CERTIFICATE OF LIABILITY INSURANCE, WHICH IN
 ACCORDANCE WITH SECTION 78-406 OF THE CITY CODE
 SPECIFICALLY NAMES THE CITY OF WEST PALM BEACH, ITS
 COMMISSIONERS, OFFICERS, EMPLOYEES AND AGENTS AS
 ADDITIONAL INSURED.
