 1) REPEAT COMMENT: ALONG CLARE AVENUE (BETWEEN
 HANDHOLES 0+00 AND 5+35), THE LATEST SURVEY SHOWS
 APPROXIMATELY THREE (3) FEET BETWEEN THE RIGHT-OF-WAY
 LINE AND THE EDGE OF PAVEMENT AS WELL AS OFFSET FROM
 THE BUILDINGS. THE ENGINEERING PLANS SHOW THE
 RIGHT-OF-WAY LINE AT THE FACE OF THE BUILDINGS WITH AN
 APPROXIMATELY SIX (6) FOOT WIDE SIDEWALK. WHILE THE
 BORE IS GENERALLY SHOWN AS FOUR (4) FEET FROM THE FACE
 OF THE BUILDINGS ON THE ENGINEERING PLAN, BASED ON THE
 SURVEY, THE BORE IS WITHIN PRIVATE PROPERTY, WHICH IS
 NOT PERMITTED UNLESS A UTILITY EASEMENT IS PRESENT. NO
 UTILITY EASEMENT IS SHOWN ON THE SURVEY. PLEASE REVIEW
 YOUR PLANS AND SHOW ACCURATE CONDITIONS. THIS NEEDS TO
 BE CORRECTED EITHER WITH A NEW SURVEY OR REVISED
 ENGINEERING PLANS.
  
 2) REPEAT COMMENT: SHEET 13A STATES THAT TIE IN 2+65
 REQUIRES THE REMOVAL AND RESTORATION OF A GRASS AREA.
 THERE IS NO GRASS IN THE RIGHT-OF-WAY ALONG CLARE
 AVENUE, SO THIS MAY NOT OCCUR AS IT WOULD BE ON PRIVATE
 PROPERTY. NOTE: SHEET 13 LISTS THE SAME TIE IN AS
 REPLACING CONCRETE. ALL PLANS MUST STATE SAME
 CONDITIONS.
  
 3) REPEAT COMMENT: PROVIDE SURVEY FOR SHEETS 22 AND 23.
 THE BORE ON SHEET 22 APPEARS TO CROSS INTO PRIVATE
 PROPERTY AND THE BORE ON SHEET 23 SHOWS ENCROACHMENT ON
 PRIVATE PROPERTY. UNLESS THERE IS A UTILITY EASEMENT
 SHOWN ON THE SURVEY AT THESE LOCATIONS, YOU MAY NOT
 CROSS ONTO PRIVATE PROPERTY. NOTE: BASED ON REVIEWING
 THE PALM BEACH COUNTY PROPERTY APPRAISER SITE, THE
 SIDEWALK IS NOT WHOLLY WITHIN THE RIGHT-OF-WAY;
 THEREFORE, YOUR BORE UNDER THE SIDEWALK CROSSES ON
 PRIVATE PROPERTY, WHICH IS NOTE PERMITTED. THE SURVEY
 IS REQUIRED TO VERIFY THAT EITHER YOU ARE NOT ON
 PRIVATE PROPERTY OR THAT THERE IS A UTILITY EASEMENT
 FOR WHERE YOU ENCROACH.
  
 4) REPEAT COMMENT: SHEETS 24 AND 25 STILL SHOW THE
 INCORRECT LOCATION OF THE RIGHT-OF-WAY AT THE NORTH END
 OF HOWARD PARK. THE AREA SHOWN AS RIGHT-OF-WAY AT THE
 BACK OF THE SIDEWALK IS PRIVATE PROPERTY OWNED BY THE
 CITY AND NOT RIGHT-OF-WAY. THE HANDHOLE, TIE IN AND
 BORE HAVE BEEN SHOWN MOVED OUT OF THE PRIVATE PROPERTY
 (BACK OF SIDEWALK); HOWEVER, THE ENGINEERING PLAN NEEDS
 TO BE CORRECTED TO SHOW THE ACCURATE RIGHT-OF-WAY
 CONDITION.
  
 5) REPEAT COMMENT: ENGINEERING SHEET 29 SHOWS A NEW
 HANDHOLE IN THE SIDEWALK ON THE NORTH AND SOUTH SIDES
 OF FERN STREET. THE CONSTRUCTION FOR THE 360 SOUTH
 ROSEMARY OFFICE TOWER HAS BEEN COMPLETED. YOU MUST
 UPDATE YOUR PLANS TO SHOW THE COMPLETED RIGHT-OF-WAY
 WORK AND VERIFY THAT HANDHOLE 99+60 DOES NOT CONFLICT
 WITH STREET TREES OR A POSSIBLE GAS LINE. ADDITIONALLY,
 THE SURVEY NEEDS TO SHOW THE SOUTH SIDE OF FERN STREET
 IN THE LOCATION OF WORK. CURRENTLY THE SURVEYOR'S NOTES
 COVER THE AREA.
  
 6) PLEASE NOTE THAT CITYPLACE RETAIL, LLC, IS IN THE
 PROCESS OF RECONFIGURING THE SIDEWALK/LANDSCAPE WITHIN
 THE OKEECHOBEE BOULEVARD RIGHT-OF-WAY AT THE LOCATION
 OF HANDHOLE 1+69. YOU WILL NEED TO VERIFY THE FINAL
 CONDITIONS TO COORDINATE YOUR INSTALLATION. STAFF IS
 CONCERNED WITH THE FINAL GRADE OF THE HANDHOLE IN
 RELATIONSHIP TO THE SURROUNDING AREA.
  
 7) THE BORE SHOWN AT 5 TO 12 FEET BELOW GRADE IS AT A
 SUFFICIENT DEPTH TO AVOID TREE AND PALM DAMAGE.
  
 8) THE INSTALLATION CONTRACTOR MUST CONTACT SUNSHINE
 811 TO VERIFY THE LOCATION OF ALL UTILITIES AND VERIFY
 ALL DEPTHS PRIOR TO CONSTRUCTION. ALL CITY UTILITIES
 MUST BE LOCATED BY SOFT DIG METHODS BEFORE STARTING THE
 WORK.
  
 9) THE RIGHT-OF-WAY USE PERMIT REQUIRES YOU TO PROVIDE
 A CERTIFICATE OF LIABILITY INSURANCE, WHICH IN
 ACCORDANCE WITH SECTION 78-406 OF THE CITY CODE
 SPECIFICALLY NAMES THE CITY OF WEST PALM BEACH, ITS
 COMMISSIONERS, OFFICERS, EMPLOYEES AND AGENTS AS
 ADDITIONAL INSURED.
