 UNDERGROUND FIBER CLARE TO CLEMATIS - 5TH REVIEW
  
 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) OUTSTANDING PLAN SHEETS REQUIRED:
  
 A. THANK YOU FOR PROVIDING THE SURVEY(S) FROM EXISTING
 HANDHOLE 0+00 TO HANDHOLE 14+85. SURVEY SHEETS 1, 2 AND
 3 DO NOT SHOW THE CLARE AVENUE AND BLANCHE STREET
 RIGHTS-OF-WAY LINE IN THE SAME LOCATION AS ON
 ENGINEERING SHEETS 13 AND 14 (LABELED AS 13). ALONG
 CLARE AVENUE THE SURVEY SHOWS AN APPROXIMATELY THREE
 FOOT WIDE SIDEWALK, WHICH WHEN COMPARED TO THE LOCATION
 OF THE BORE ON THE ENGINEERING PLAN HAS THE BORE
 CROSSING OUT OF THE RIGHT-OF-WAY TO PRIVATE PROPERTY IN
 A FEW INSTANCES. NO UTILITY EASEMENT IS SHOWN ON THE
 SURVEY TO PERMIT THE INSTALLATION ON PRIVATE PROPERTY.
 PLEASE REVIEW YOUR PLANS AND SHOW ACCURATE CONDITIONS.
 THE BLANCHE STREET FIBER BORE IS SHOWN WITHIN THE AREA
 THAT IS SHOWN AS RIGHT-OF-WAY ON THE SURVEY.
  
 B. SHEET 13A STATES THAT TIE IN 2+65 REQUIRES THE
 REMOVAL AND RESTORATION OF A GRASS AREA. THERE IS NO
 GRASS IN THE RIGHT-OF-WAY ALONG CLARE AVENUE, SO THIS
 MAY NOT OCCUR AS IT WOULD BE ON PRIVATE PROPERTY. NOTE:
 SHEET 13 LISTS THE SAME TIE IN AS REPLACING CONCRETE.
 ALL PLANS MUST STATE SAME CONDITIONS.
  
 C. REPEAT COMMENT: PROVIDE SURVEY FOR SHEETS 22 AND 23.
 THE BORE ON SHEET 22 APPEARS TO CROSS INTO PRIVATE
 PROPERTY AND THE BORE ON SHEET 23 SHOWS ENCROACHMENT ON
 PRIVATE PROPERTY. UNLESS THERE IS A UTILITY EASEMENT
 SHOWN ON THE SURVEY AT THESE LOCATIONS, YOU MAY NOT
 CROSS ONTO PRIVATE PROPERTY. ***** REMOVING THE PAGES
 DOES NOT ADDRESS WHETHER OR NOT YOU ARE CROSSING ONTO
 PRIVATE PROPERTY.
  
 D. REPEAT COMMENT: SHEETS 24 AND 25 STILL SHOW THE
 INCORRECT LOCATION OF THE RIGHT-OF-WAY. THE HANDHOLE,
 TIE IN AND BORE HAVE BEEN SHOWN MOVED OUT OF THE
 PRIVATE PROPERTY (BACK OF SIDEWALK); HOWEVER, THE
 ENGINEERING PLAN NEEDS TO BE CORRECTED TO SHOW THE
 ACCURATE CONDITION. ***** REMOVING THE PAGES DOES NOT
 CORRECT THE INCORRECT DEPICTION OF THE RIGHT-OF-WAY
 LOCATION ON THE ENGINEERING PLANS.
  
 E. ENGINEERING SHEET 29 NOW SHOWS A NEW HANDHOLE IN THE
 SIDEWALK ON THE NORTH AND SOUTH SIDES OF FERN STREET. A
 REVIEW OF THE APPROVED LANDSCAPE PLAN FOR THE NEW 360
 SOUTH ROSEMARY OFFICE TOWER INDICATES THAT THERE IS A
 DIRECT CONFLICT WITH THE PROPOSED HANDHOLE ON THE NORTH
 SIDE OF FERN STREET AND A STREET TREE AS WELL AS A GAS
 LINE. NEITHER THE SURVEY OR THE ENGINEERING PLAN SHOW
 THE GAS LINE IN THE SAME LOCATION AS THE LANDSCAPE
 PLAN. IT APPEARS THAT THE GAS LINE IS BEING RELOCATED
 AS PART OF THE PROJECT. YOU MUST SHOW THE FINAL
 CONDITIONS AND DESIGN YOUR INSTALLATION TO NOT CONFLICT
 WITH THE FINAL CONDITIONS. ADDITIONALLY, THE SURVEY
 NEEDS TO SHOW THE SOUTH SIDE OF FERN STREET IN THE
 LOCATION OF WORK. CURRENTLY THE SURVEYOR?S NOTES COVER
 THE AREA.
  
 2) PLEASE NOTE THAT CITYPLACE RETAIL, LLC, IS IN THE
 PROCESS OF RECONFIGURING THE SIDEWALK WITHIN THE
 OKEECHOBEE BOULEVARD RIGHT-OF-WAY AT THE LOCATION OF
 HANDHOLE 1+69. YOU WILL NEED TO VERIFY THE FINAL
 CONDITIONS TO COORDINATE YOUR INSTALLATION.
  
 2) THE BORE SHOWN AT 5 TO 12 FEET BELOW GRADE IS AT A
 SUFFICIENT DEPTH TO AVOID TREE AND PALM DAMAGE.
  
 3) WHEN RESUBMITTING THE PLANS, INCLUDE THE PERMIT
 NUMBER (19020053) AND A WRITTEN RESPONSE TO THE
 COMMENTS.
  
 4) THE INSTALLATION CONTRACTOR MUST CONTACT SUNSHINE
 811 TO VERIFY THE LOCATION OF ALL UTILITIES AND VERIFY
 ALL DEPTHS PRIOR TO CONSTRUCTION. ALL CITY UTILITIES
 MUST BE LOCATED BY SOFT DIG METHODS BEFORE STARTING THE
 WORK.
  
 5) THE RIGHT-OF-WAY USE PERMIT REQUIRES YOU TO PROVIDE
 A CERTIFICATE OF LIABILITY INSURANCE, WHICH IN
 ACCORDANCE WITH SECTION 78-406 OF THE CITY CODE
 SPECIFICALLY NAMES THE CITY OF WEST PALM BEACH, ITS
 COMMISSIONERS, OFFICERS, EMPLOYEES AND AGENTS AS
 ADDITIONAL INSURED.
