 UNDERGROUND FIBER CLARE TO CLEMATIS - 2ND REVIEW
  
 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) ENGINEERING PLANS
  
 A. UPDATED COMMENT: THE ENGINEERING PLANS MUST SHOW THE
 RIGHT-OF-WAY IMPROVEMENTS FOR THE CLARE AVENUE
 RESIDENTIAL PROJECT (CLARE AND WILKINS AVENUES), THE
 ROSEMARY AVENUE RECONSTRUCTION PROJECT, THE 360 SOUTH
 ROSEMARY PROJECT (SOUTH ROSEMARY AVENUE AND FERN
 STREET), THE QUADRILLE/FERN MAST ARM INSTALLATION
 PROJECT AND THE CLEMATIS STREET PHASE III PROJECT.
 STAFF NEEDS TO VERIFY THAT THE PROPOSED FIBER CONDUIT,
 MARKER POLES, HANDHOLDS AND FUTURE COMMUNICATION POLES
 DO NOT INTERFERE WITH THESE APPROVED RIGHT-OF-WAY
 PROJECTS.
  
 2) UPDATED COMMENT: SURVEY OF UNDERGROUND FACILITIES -
 THE SURVEY NEEDS TO BE AT A SIZE/SCALE THAT IS LEGIBLE.
 STAFF IS UNABLE TO MAKE OUT ANY DETAILS ON THE SURVEY
 THAT WAS PROVIDED.
  
 A. ALL UNDERGROUND UTILITIES NEED TO BE PROVIDED ON THE
 SURVEY. IF THE BORE IS PROPOSED WITHIN FIVE (5) FEET OF
 AN EXISTING UTILITY, YOU NEED TO PROVIDE A LETTER FROM
 THAT UTILITY THAT THE PROPOSED BORE DOES NOT INTERFERE
 WITH THE PROVISIONS OF THEIR SERVICES.
  
 B. THE SURVEY, PREPARED BY A FLORIDA LICENSED SURVEYOR,
 MUST SHOW THE RIGHT-OF-WAY LINE, THE SIDEWALK, AND ANY
 TREES WITHIN THE RIGHT-OF-WAY ALONG THE FIBER CONDUIT
 PATH.
  
 C. THE ENGINEERING PLANS MUST SHOW THE SAME CONDITIONS
 AS THE SURVEY, SO THAT STAFF CAN VERIFY THAT NONE OF
 THE PROPOSED WORK (MARKER POLES, HANDHOLDS OR CONDUIT
 IS BEING INSTALLED ON PRIVATE PROPERTY AND THAT NO
 TREES (SPECIFICALLY THEIR ROOT SYSTEM) ARE BEING
 AFFECTED.
  
 D. PROVIDE THE SURVEY BACK UP FOR SHEETS 19 THROUGH 22,
 SO THAT STAFF CAN VERIFY THAT THE PROPOSED WORK IS IN
 THE RIGHT-OF-WAY. THE LOCATION MAP LINE APPEARS TO
 CROSS OUT OF THE RIGHT-OF-WAY LINE ONTO PRIVATE
 PROPERTY.
  
 3) REPEAT COMMENT: ESTIMATED SCHEDULE - NO SCHEDULE WAS
 PROVIDED. PROVIDE A NARRATIVE OF THE ESTIMATED SCHEDULE
 OF THE PROPOSED WORK.
  
 4) POLE STYLE - PLEASE NOTE THAT THE FUTURE POLES ARE
 REQUIRED TO MEET THE CITY CONCEALMENT STANDARDS
 OUTLINED IN SECTION 78-411 OF THE CITY CODE.
  
 NOTE: STAFF WILL NOT APPROVE THE INSTALLATION OF A SIX
 (6) FOOT TALL MARKER POLE ADJACENT TO THE HANDHOLES. IF
 THE HANDHOLES NEED TO BE MARKED, THEN PROVIDE THE
 INFORMATION ON THE HANDHOLE COVER.
  
 5) ELECTRONIC FORMAT OF SUBMITTAL -. A COPY OF ALL OF
 THE SUBMITTAL MATERIAL MUST BE SUBMITTED IN ELECTRONIC
 FORMAT.
